Cincinnati Medical Home Initiatives • Began in October 2009 • 11 practices with full facilitation & PMPM funding • 9 practices with partial facilitation • 17 NCQA recognized (13 at Level 3), 3 awaiting feedback • Transformation assisted by TransforMED with specific emphasis on culture & philosophy as well as changes in process & structure

  2. Challenges: • Complexity of task • Focus over time • Maintenance of enthusiasm • Adequate resources (intellectual capital as well as funding)

  3. Expansion Plans: • 20 new practices to come online in April 2011 • More emphasis on up-front baseline data collection regarding quality, practice efficiency, staff & patient satisfaction • Will augment external transformation input (TransforMED) with onsite practice coaches
