  1. Health Concerns Jefferson Band of Blue San Diego Trip Laura Wheeler, RN BSN M.Ed NCSN CRCSD School Nurse

  2. Complete Health Form • Return by December 18th • List any medications your student takes whether over the counter (OTC) or prescription • List any health conditions • Check the box on the back if you would like your child to receive OTC medication in the event of need

  3. Special Health Conditions • Please see me after the meeting • Students with the following: • Diabetes • Asthma • Allergic Reactions/Anaphylactic Reactions • Seizures • Other chronic health conditions

  4. Items to Send • Sunscreen ( Should be at least 30 SPF) • Small First Aid Kit • Band-Aids • Antibiotic Ointment • Tylenol or Advil in original bottles (only few needed) • Small bottle of hand sanitizer

  5. Medication • Prescription medication should be in the original bottle with the correct prescription label. • Only send amount needed for the trip. • OTC medications need to be in their original containers • No unlabeled medications will be allowed

  6. Head Chaperone of Group • Will have First Aid Kit in a Fanny Pack • They will have a card with questions to ask before giving any medication to a student • Questions include: • Are you taking any medication? • Do you have any allergies to medications? • Do you have high blood pressure? • Do you have Diabetes? • Do you have glaucoma? • Do you have Asthma? • Do you have any urinary problems? • Have you taken this medication before?

  7. Continued • OTC meds may include: • Tylenol, Motrin, Generic Antihistamine, Generic decongestant/antihistamine, Cough Medication, Tums, Pepto Bismal Tabs, Motion Sickness, Anti-diarriheal

  8. Consider Checking with Your Doctorabout: • Having your student get a seasonal Influenza vaccine (flu shot) • Having your student get an H1N1 Vaccine • Receiving the vaccine for Meningitis • Receiving the booster vaccine for Pertussis (Boostrix)

  9. Encourage Your Student • To get enough rest before leaving • Eat healthy meals including fresh fruits/vegetables • Drink water to stay hydrated • Practice Good Handwashing!!!!!! • Covering coughs/sneezes
