2. Consortiwm Arloesi Teuluoedd yn Gyntaf Gorllewin Cymru / West Wales Families First Pioneer Consortium 2011-12 Sir Gaerfyrddin Ceredigion Sir Benfro ar y cyd gyda Bwrdd Iechyd Hywel Dda Carmarthenshire Ceredigion Pembrokeshire in conjunction with Hywel Dda Health Board

Strategaeth Tlodi Plant Cymru / Child Poverty Strategy for Wales • Gostwng nifer y teuluoedd lle nad oes unrhyw un yn gweithio • Gwella sgiliau rhieni/gofalwyr a phobl ifanc sy'n byw mewn aelwydydd incwm isel, er mwyn iddynt allu sicrhau cyflogaeth sy'n talu'n dda • Lleihau'r anghydraddoldebau sy'n bodoli o ran canlyniadau iechyd, canlyniadau addysg a chanlyniadau economaidd plant a theuluoedd, trwy wella canlyniadau'r rhai tlotaf • To reduce the number of families living in workless households • To improve the skills of parents/carers and young people living in low-income households so they can secure well-paid employment • To reduce inequalities that exist in health, education and economic outcomes of children and families by improving the outcomes of the poorest

6. Gwasanaethau cyffredin ond… /Common services but... Nid yw pob system yr un fath, felly bydd dull gweithredu rhanbarthol tuag at gyflawni yn sicrhau y byddai gan ein hardaloedd trefol a gwledig ddull gweithredu mwy cydlynol o ran y gwasanaeth Not all systems are the same therefore a regional approach to delivery will ensure that our urban and our rural areas would have a more coherent service approach

Ein Gweledigaeth / Our Vision Dull gweithredu integredig ac sy'n canolbwyntio ar y teulu cyfan tuag at ddarparu gwasanaethau ar draws yr holl asiantaethau, sy'n sicrhau bod gwasanaethau cyffredinol, ataliol ac ymyrraeth gynnar yn cael cymaint o effaith ag y bo modd ac yn diwallu anghenion teuluoedd a phlant Integrated, whole family approach to service delivery maximising impact of universal , preventative & early intervention services & addressing needs of families and children

Mae angen i ni / We need to • Ddatblygu rhyngwyneb cadarn rhwng systemau, adrannau ac asiantaethau o fewn siroedd ac ar draws y 3 sir. • Cyflymu gweithgareddau er mwyn datblygu'r gweithlu ac arferion rhannu gwybodaeth ar draws y dair sir. • Develop robust interface between systems, departments & agencies inter-county and across the 3 counties. • Speed up workforce development activities & information sharing practices across the three counties.

Mae angen i ni / We need to • Ddatblygu'r cyswllt gyda defnyddwyr er mwyn deall argraffiadau defnyddwyr yn well ynghylch hwylustod wrth fanteisio ar wasanaethau, ac er mwyn ystyried ffyrdd o gryfhau gweithgarwch ymgysylltu gyda defnyddwyr. • Pennu proses er mwyn cael ein herio gan ein gilydd fel 'cyfeillion beirniadol'. • Enhance user involvement to better understand users’ perceptions of ease of access and to explore ways to strengthen user engagement. • Establish a process for receiving challenge from each other as ‘critical friends’.

Cynllunio Gwasanaethau ar gyfer yr Anabl / Disability Services Planning • Nodi'r newidiadau y mae angen eu gwneud er mwyn sicrhau bod mwy o debygrwydd rhwng gwasanaethau, bod safonau gwasanaeth cyffredin yn bodoli a bod adnoddau yn cael eu darparu ar gyfer plant anabl ar draws y rhanbarth • Datblygu timau aml-ddisgyblaethol sefydledig er mwyn cynorthwyo plant anabl, gan gryfhau a datblygu rôl y gweithiwr allweddol er mwyn iddo fod yn effeithiol ar draws ffiniau sirol • Identify required changes to ensure greater similarities of service, common service standards & resource provision for disabled children across region • Build on established multi – disciplinary teams to support disabled children, strengthening & developing key worker role to be effective across county boundaries

Ein Dull Gweithredu / Our Approach • Cryfhau ein darpariaeth Tîm o Gwmpas y Teulu (TAF) trwy dreialu arfer arloesol yn ardaloedd yr awdurdodau • Dysgu o'n gilydd a chynllunio ar gyfer dull gweithredu rhanbarthol ar y cyd tuag at ddarparu • Strengthening our Team around the Family(TAF) provision through the trialling of innovative practice in each of the authority areas • Learning from each other and planning for a move towards a collaborative regional approach to delivery
