What are our Objectives? • To improve the quality of life for residents living in Torbay care homes • To support care homes to improve the quality of care for their residents. • Maintain links and ensure good working relations with care homes. • To monitor and audit quality standards for care homes and highlight concerns/good practice.

  4. Who do we work with? • Care home owners, managers, staff, residents and their families. • Health and social care front line staff/teams. • Review and Reassessment Team. • Specialist professionals/departments. • CQC. • Commissioners. • Healthwatch. • Other Local Authorities. • CCG (Yellow card).

  5. What do we do? • Monitor and follow up concerns. • Carry out audits/follow up results. • Signpost/advice/guidance. • Attend safeguarding meetings. • Heavy involvement in provider of concern process. • Help implement and monitor service improvement plans. • Training/education. • Practical visits. • Monthly quality report. • Maintain Care Home Resource and Information Site (CHRIS).
