  1. CYBER Face Sheet Printing December 21, 2010

  2. Training Purpose To review the new printing functionality available for users that have access to a youth/child’s Face Sheet in CYBER.

  3. Availability Face Sheet Printing will be available to any CYBER user that has access to a youth/child’s record (user is open in Tracking Elements). This function is not currently available via Historical Access.

  4. To access the Print function, search for the youth/child’s CYBER record by; • Accessing Your Caseload from the Welcome Page or, • Using the Quick Search function on the Welcome Page or, • Accessing the Youth/Child Search from the Welcome Page.

  5. Print Face Sheet Found on the Demographics tab, in the lower right side of the screen.

  6. Report Elements The following elements of the Face Sheet will print in the report; • Demographics • Admissions • Providers • Tracking Elements • Formal/Informal Supports • Eligibility * Diagnosis is also included on the report.

  7. Keep in Mind….. Depending upon the amount of information contained in the record, it may take a few moments for the report to load.

  8. The report will appear in the View Report tab. • If the report is slow to load, or does not completely load, there is a refresh button that can be used to reload the report.

  9. Using the Export to Selected Format menu, the report can be exported into four formats; • Adobe PDF file • Excel spreadsheet • Rich Text Format (can be used in Word) • TIFF file (can be faxed from the user’s PC)

  10. Printing Report is printable right from CYBER. Print button

  11. The Demographics area of the report includes all of the details from the Demographics tab of the Face Sheet; including Additional Address Information, past Living Situations, Additional Contact Numbers, and Languages Spoken.

  12. The Diagnosis area will list the most current data that has been entered and will include all Axis fields, if applicable. The Admissions area will list all information, in descending order. (Admit and Discharge Reasons will not be included.)

  13. The Provider area will include the most current only; those listed in CYBER with no end date. The Tracking Elements area will list all Tracking Elements, in ascending order. The Formal/Informal Supports will include all Supports listed in the Face Sheet, in alphabetical order. The Eligibility area will list the child’s most current Medicaid Eligibility information, with the most recent listed first.
