  3. Patient Identification Always use Two Patient Identifiers: Name Date of Birth History Number Inpatients: Check Name and History Number on patient’s wrist band with the label. Outpatients: Patient states Name and Date of Birth while you compare Labels and Requisition. Always label containers (Blood, Urine, etc.) in the presence of the patient. Never use the patient’s room as a patient identifier.

  5. Reduce Health Care Associated Infections Direct Patient Interaction Natural nails less than 1/4-inch long. Wash hands in the presence of the patient. Laboratory Wash hands after removing gloves and lab coats. Wash hands before eating. Use alcohol-based hand rub.

  6. Patient Involvement Active patient identification Encourage patients to ask questions Refer patient to physician or nurse when appropriate.
