Review alerts in HCSIS for considerations submitted by the IMT (Independent Monitoring Team) • Communicate consideration with the appropriate team member but not limited to the following: consumer, family, and provider • Respond to consideration in HCSIS within 45 days • Upon the request of the AE, revise consideration in HCSIS • HCSIS will send alerts every 10 days for consideration that need to be addressed SCO ROLES WITH CONSIDERATIONS

Consideration: Consumer stated he wants to be a school teacher. • Consideration response: The consumer does not have the ability to be a school teacher. • The supports coordinator needs to “think outside the box” and be creative in approaching considerations. • Also, if the consideration phrase begins with “Consumer stated” the supports coordinator must state in the considerations response that the consideration was discussed with the consumer. Example of a consideration response that would not be approved by AE

  5. Satisfactory action taken in regards to considerations What is closing the LOOP?

  6. Group #1- Individual satisfaction • Consumer/Spokesperson satisfaction with the responses or actions. Group #2- IM4Q/AE satisfaction • Satisfaction with the response. Satisfied with the details given. The who, what, where, when and why. What two groups should be satisfied?

  7. Ensure that providers are in the “loop” of the process of considerations, signs of quality, and major concerns that will be entered and addressed in HCSIS Coordinate and meet with providers to address/resolve considerations and major concerns SCO ROLES WITH providers
