  2. Rural Health Clinic (RHC) An RHC is a facility located in a rural area designated as a shortage area and is neither a rehabilitation agency nor a facility primarily for the care and treatment of mental diseases. There are currently 63 RHC’s in Indiana

  3. Condition for Coverage: Compliance with Federal, State, and Local Laws(J0003, J0004, J0005) Federal Laws and Regulations- Conditions for certification under federal regulations are the subject of these guidelines State Laws and Regulations- State Practice Acts

  4. Condition for Coverage: Location of Clinic (J0006, J0007, J0008, J0009, J0010, J0011, J0012, J0013, J0014, J0015, J0016, J0017, J0018) The clinic must be located in a rural area that is designated as a shortage area Applicants should contact Ray Guest, Primary Office Manager, of Partner Relations, ISDH- (317)233-7846 or rguest@isdh.in.gov Mobile Units

  5. Condition for Coverage: Physical Plant and Environment (J0019, J0020, J0021, J0022, J0023, J0024, J0025, J0026, J0027, J0028) Physical plant maintained consistent with appropriate State and local building, fire, and safety codes. Preventative maintenance program Mechanical, electrical, patient-care equipment, drug and biological storage Non-medical emergency procedures

  6. Condition for Coverage: Organizational Structure (J0029, J0030, J0031, J0032, J0033, J0034, J0035, J0036) Clinic required to be under medical direction of physician(s) Written policies (administrative and patient care) Disclosure of names and addresses

  7. Condition for Coverage: Staffing and Staff Responsibilities (J0037, J0038, J0039, J0040, Joo41, J0042, J0045, J0046, J0047, J0048, J0049, J0050, J0051) Sufficient staffing Staffing availability- a physician, nurse practitioner, certified nurse midwife or physician assistant must be available to furnish patient care services at all time the clinic operates A nurse practitioner, certified nurse midwife or physician assistant must be available to furnish patient care at least 50% of the scheduled operating hours during which RHC services are being offered Staff responsibilities

  10. Condition for Coverage: Program Evaluation (J0076, J0077, J0078, J0080, J0081, J0082, J0083, J0084, J0085, J0086) Annual total program evaluation Evaluation review criteria Purposes of annual evaluation Evaluation findings and action taken
