  5. PHYSICALS/PAPERWORK • PHYSICALS DEADLINE: FRIDAY, NOVEMBER 22nd • NO PHYSICAL/PAPERWORK = NO TRY-OUT • MEDICAL HISTORY – (Parent Fills Out/Dr. Reviews) • PHYSICAL – (DOCTOR) • CONCUSSION • RULES ACKNOWLEDGEMENT/PARENT PERMIT • RULES/ELIGIBILITY • KITTY HAWK RULES

EQUIPMENT • Players responsible for issued equipment • Locker & Padlock issued • Athletes responsible for clothing cleanliness • Game jerseys turned in after each game • Only School issued equipment allowed • Proper uniform required for each practice & game (No cut off sleeves, frayed shorts, etc.)

  7. TRAVEL • PLAYERS WILL TRAVEL TO/FROM GAMES WITH COACHES ON SCHOOL BUSES • EXCEPTIONS MAY BE MADE IF REQUEST IS SUBMITTED IN WRITING TO THE HEAD COACH AT LEAST 1 DAY PRIOR TO THE GAME GO FLYERS!!! WIN S.T.A.C.!!

ACADEMICTRY-OUT • NO PASS, NO PLAY (STATE LAW) • MANDATORY WEEKLY GRADE CHECKS- PASSING, SATISFACTORY, COMPLETE, & ON TIME • GRADES - 70 & Above ALL CLASSES • CONDUCT – “S” AND “E” • REPORT CARD – 1 FAILING GRADE – 3 WEEKS INELIGIBLE (State Law) • TUTORING/STUDY HALL 3:30- 4:05 PM • ABSENT – SEE HEAD COACH • FORGETFULNESS UNACCEPTABLE

  9. TEAM ROSTER NUMBERS • 7TH GRADE – 30 total • 8TH GRADE – 25 total • FRIDAY, DECEMBER 6th :TEAM ROSTERS POSTED OUTSIDE ON GYM DOORS • First Practice on DEC. 9TH

  10. ATHLETIC TRY-OUT • MANDATORY ATTENDANCE – Required each session • EXCUSED ABSENCE – Head Coach must be notified • UNEXCUSED ABSENCE – AFFECTS Athletic Class Grade, Try-outs & Playing time • MAY BE BASIS FOR DISMISSAL FROM THE TEAM.
