Accepted For Value some initial things to remember as you choose which process to use... • Is the presentment a recurring bill? • Does the presentment have a coupon or is this presentment a full page statement? • Does the “bill presenter” already have my SSN in their system? • Do I wish to settle the account and close the account upon sending the AFV? • Am I trying to get monies back from this AFV if it is my bank or debit/credit card?

Example One…Recurring Bill or Recurring Statement • Usually your electric, gas, water, utilities, phone, cell phone or credit card bill. *(Usually comes to you monthly and could be a mortgage note as well). • Usually a perforated bill with a coupon at the bottom… sometimes it is at the top and sometimes it has a dotted line dividing the statement summary and the coupon. • Usually has a monthly payment and a total bill payoff amount. • Most common of the bill presentments.

Example #1 – Sample A ABC ELECTRIC COMPANY • Notice Type of Bill. • Notice Total Monthly Payment Due. • Notice Statement at the Top of Bill and the Coupon at the Bottom of the Bill. • Notice Perforations to Allow Bill to be Separated. • Stamp “Accepted For Value and Return For Value” at 45 Degree Angle on the Statement Part of the Bill in “Red Ink”. • “Write-In” the Amount You Want to be Utilized to Pay the Bill. *(Make the Amount a little more than the Payment… Discretion Needs to be Used). • Sign Bottom of Coupon in “Blue Ink” with “Authorized Representative” Written Directly Underneath Your Signature. Total Monthly Payment....... $235.00 ACCEPTED FOR VALUE AND RETURN FOR VALUE $350.00 Payment Enclosed… Sick N. Tired Authorized Representative FRONT OF STATEMENT/BILL

Example #1 – Sample A ABC ELECTRIC COMPANY • Now Separate or Detach the Coupon from the Bill Statement and Flip the Coupon Over and Endorse the Coupon on the Back, Just Like You Would Endorse a Check… Place Your Signature on the End of the Coupon on the Left Hand Side On the Back of the Coupon. • *This Procedure of Endorsing the Coupons, will be the same Throughtout this System of Processes. $350.00 Payment Enclosed… Sick N. Tired Authorized Representative BACK OF COUPON

  7. Example #1 – Sample B JFK CREDIT CARD COMPANY • This Credit Card Presentment will Follow the Same Process as Example One With the Statement and Coupon. The Only Difference will be That You now Have a Total Payoff Amount that You will Add to the Total Monthly Payment and Write the Sum Total in the Payment Enclosed Area on the Coupon. You Would Detach and Endorse the Coupon the Same Way You did in Example One. Total Monthly Payment....... $235.00 Total Payoff...... $7,852.78 ACCEPTED FOR VALUE AND RETURN FOR VALUE $8,078.78 Payment Enclosed… Sick N. Tired Authorized Representative FRONT OF STATEMENT/BILL

  8. Example Two…Settlement and Closure • Usually your car payment, loan payment or a mortgage payment that you wish to payoff and close. Anotherwords, you don’t want a monthly note or loan payment… ANYMORE! • Usually a perforated bill with a coupon at the bottom… sometimes it is at the top and sometimes it has a dotted line dividing the statement summary and the coupon. *Hint- (If you have a coupon book, request a statement with a payoff amount on it.) • Usually occurs as a monthly payment with a total loan or note payoff amount.

  9. Example #2 TUV CAR LOAN COMPANY • This Loan Payment Presentment will Follow the Same Process as Example One With the Statement and Coupon. There will be (2) Differences… • 1) Your AFV Stamp has some additions; Settlement and Closure with a Date and Your Exemption Number with your “Wet Ink” Signature; *(blue ink), and • 2) The Payment Amount is Duplicated. Now You Combine the Payment With the Total Loan Payoff and Write the Sum Total in the Payment Enclosed Area on the Coupon. You Still Write Your Signature at the Bottom of the Coupon With Authorized Representative Below Your Signature.   10. ACCEPTED FOR VALUE AND RETURN FOR VALUE FOR SETTLEMENT AND CLOSURE DATE_________ EXEMPTION#___________________ By____________________________ ROUTING NUMBER BOND NUMBER The Date Sick N. Tired To fill out the “Settlement and Closure” AFV Stamp, just follow the lined areas for completion. Fill in the “Date” Line as specified, then in the “Exemption” Line write-in the Routing Number of the appropriate Federal Reserve Bank; *(The letter in front of your Bond Number on the back of your SSN Card will designate which routing number you will utilize). Then leave a space and write-in the Bond Number on the back of your SSN Card. Then, fill in the “By” Line with your “Wet Ink” Signature. That’s just your signature in the “By” Line with blue ink. Utilize blue in the date an Exemption Lines as well. This format will be utilized for all AFV Stamps that have the “Exemption” Line include with them.

Example Three…SSN Number Not Known By Bill Presenter. • Usually a Third Party Collection Agency or even a Title Loan or a Company and you never gave them your SSN Number. • Usually a perforated bill with a coupon at the bottom… sometimes it is just a dotted line that you have to cut to detach the coupon. • This presentment occurs one time and they are attempting to collect from you with no monthly payment. They usually have a payoff amount on the statement that you have.

Example #3 COLLECTION AGENCY COMPANY • This Loan Payment Presentment will Follow the Same Process as Example One and Two With the Statement and Coupon. • Follow previous instructions for the AFV Stamp for “Exemption” and fill in accordingly. • The Total Loan Payoff is the Payment Amount Enclosed. You Still Write Your Signature at the Bottom of the Coupon With Authorized Representative Below Your Signature. Total Loan Payoff...... $15,221.83 ACCEPTED FOR VALUE AND RETURN FOR VALUE DATE_________ EXEMPTION#_________________ By____________________________ $15,221.83 Payment Enclosed… Sick N. Example Four…Creating Currency With a Green Dot, Debit Card or Interest Fee Based Cards • These Debit/Credit Cards, like “Green Dot” are found at places like Walmart, Radio Shack, Walgreens, K-Mart, etc.; anywhere they may sell Visa or Master Card Debit/Pre-Paid Cards. • These cards usually have a statement that they send you via mail or you can see and print out the statement via your online back-office or at a pre-determined website. • You can AFV the statements after receiving them via mail or online by printing out the statements or requesting the statements from the card providers.

  16. Example #4 MNR GREEN DOT/DEBIT OR PRE-PAID CARD COMPANY • This Statement does not have a coupon. You print this off of the website, back-office or request a copy from the Card Company. Make a duplicate copy for your records. • You use the simple AFV Stamp for this one and you cross out the balance amount and write-in the amount you wish to be placed into this account to be utilized. • Make sure and sign your signature and directly below the signature write in the authorized representative. Card Account Balance....... $112.43 $2,500.00 ACCEPTED FOR VALUE AND RETURN FOR VALUE At the bottom of the statement on the left hand side, write in the Federal Reserve Routing No., Then space and then write in the Bond No. on the back of the SSN Card.   17. Example #4 TOP OF BACK • This is the back of the Statement, so endorse the statement just like a check the same way we have endorsed the coupons with our signature. Remember to write the signature in blue ink and write it in the middle of the back of the statement. It’s that simple. Signature is written in the middle on the back of statement. Sick N. Example Five…Full Page Statement… No Coupon, With and Without the “One Dollar” Attached for Presentment • These are full page statements usually coming as notices that could be from past loans, collection agencies, finance companies, discount stores, banks, etc.. • You have two options when you utilize this method using a one dollar bill with the statement or utilizing the statement itself, copying it and then making the copy the coupon for presentment along with the original statement. In this case, it would be good to make two copies, so that you can keep one copy for your own personal records. The other copy will be used for the coupon. The original statement will be stamped with the AFV Stamp and then the copy is signed on the bottom right hand side. Then we will endorse the back of the copy in the middle and on the side, as we have done on other examples in this process. The endorsement allows the company to release the funds via your signature.

  19. Example #5– Sample A ALK FINANCE COMPANY • This Statement does not have a coupon and the bill reaches cover to cover. You are returning this presentment to payoff a bill. • You use the simple AFV Stamp for this one and attach a “One Dollar Bill” to the top center of the statement and “paper-clip” the money to the statement. • Make sure and sign your signature and directly below the signature write in the authorized representative.   20. Example #5 - Sample A TOP OF BACK • This is the back of the Statement, so endorse the statement just like a check the same way we have endorsed the coupons with our signature.   21. Example #5– Sample B ALK FINANCE COMPANY • This Statement does not have a coupon and the bill reaches cover to cover. You are returning this presentment to payoff a bill. • You use the simple AFV Stamp for this one and since you do not have a “One Dollar Bill”, make a copy of the statement and turn the copy of the statement into the coupon for this presentment. • Make sure and sign your signature and directly below the signature write in the authorized representative. SOMEWHERE, STATE 37651 REMEMBER TO SEND AFV TO THE CORPORATE HEADQUARTERS OF THE COMPANY WHO SENT THE PRESENTMENT. USE THE “PRIORITY/CONFIDENTIAL” STAMP UNDER THE RETURN ADDRESS AND MAKE SURE THE LETTER IS CERTIFIED.

NOW FOR THE REAR OF THE ENVELOPE… THIS IS HOW IT SHOULD LOOK WHEN YOU MAIL THE PRESENTMENTS BACK FROM WHENCE THEY CAME… Back of Envelope CERTIFIED RETURN RECEIPT CARD ATTENTION CFO CONSEQUENTLY, ON THE BACK OF THE ENVELOPE AFTER YOU SEAL IT, YOU WILL THEN STAMP THE BACK RIGHT IN THE “V”OF THE ENVELOPE WITH THE ”ATTENTION CFO” STAMP. THEN YOU WILL ATTACH THE CERTIFIED RETURN RECEIPT CARD OVER THE ”ATTENTION CFO” STAMP. THE LETTER IS READY TO DO IT’S WORK FOR YOU NOW.

These are the AFV Stamps that you need to have for this process… • There are (7) stamps in this AFV process that you will utilize as you deal with these presentments. • You will also need a red ink pad to use your own thumbprint as your “living seal” from time to time.
