Harmonisation & Service Packs ProjectPost Implementation Review (Phase 2) • Questionnaire To Be Issued to Participants WC 19/08/2013: • Questionnaire to cover two distinct areas: 1: Planning, information and support provision during the immediate ‘Go-Live’ period 2: Participant experience of the period following ‘Go-Live’ to date. • Undertaking a review seeking to identify problems that followed ‘Go-live’ to include: • a. The nature of the problems • b. The actions taken to identify root causes • c. The root causes themselves • d. The corrective actions that have been put in place or are planned • e. At what point(s) there was an opportunity to avoid the problems • f. Why the problems were not identified in testing
