Mission Statement It is our goal to protect and preserve your health and your family’s health.  In addition to primary care, our staff offers expertise in the complete spectrum of family-related health, including children’s immunizations and school physicals, women’s health care and geriatric care. Kristin Atherton, M.D., FAAFP is a graduate of the University of Oxford, United Kingdom, and emigrated to the U.S., where she completed her residency at Kaiser Permanente Hospital in Harbor City, CA. Confidentiality Statement All patients have certain rights to their privacy under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).  This privacy protects your protected health information (PHI) and covers the following: Treatment – direct and indirect treatment by other healthcare providers involved in your treatment. Payment – obtaining payment from your insurance company. Operations – the day-to-day health care operations of our practice. All patients will be given the right to review and secure a copy of their Statement of Privacy Practices.  This notice contains a more detailed description of the uses and disclosures of your protected health information.  We reserve the right to change the terms of this notice from time to time and, when contacted, will disclose it to our patients. As a patient, you have the right to request restrictions on how your information is used and disclosed to carry the treatment, payment, and health care operations.  You may revoke consent, in writing, at any time.  However, use or disclosure that occurred prior to the date consent was revoked is not affected.  Your physician and our staff understand the importance of patient confidentiality and are committed to the protection of your personal health information. Payments made be paid by cash, check, and debit or credit card. Valid identification is required for check and card payments. Copayments are due at time of service unless prior arrangements have been made.
