  2. Jun 07 – Jun 13 Medical #14 in 2014 Kent June 8, 2014 EE was removing aluminum fines from NF1 knockout using a rag dipped in copper sulfate solution. Tail of the rag in EE's hand contacted discharge air stream, solution was blown out and a small amount got under safety glasses & into his eye. Practice was immediately stopped and proper cleaning practices developed.

  3. Jun 07 – Jun 13 Whitehouse June 9, 2014 A pallet in the universal storage room was found to have alcohol, acid & base drums on it. These must be stored separately. Found that an ITO had put it in this room when delivery arrived earlier that day. Acid & base drums were moved to chemical storage locker in their respective areas. Alcohol drums in the universal storage room were grounded and consolidated with other alcohol drums. Counseled the driver on inspecting/sorting and contacted vendor.

  4. Jun 07 – Jun 13 Whitehouse June 11, 2014 CPO notified supervisor of a malfunctioning WWTcrane for handling filter press screens. Down button worked intermittently before failing and hoist sliding assembly had excessive resistance preventing, making it hard to move. CPO stopped using crane, load set to rest and vendor was contacted for service.

  5. Jun 07 – Jun 13 Valparaiso June 12, 2014 EE was helping to loosen a bolt on M5 tab feeder that was being used to aid in the reattaching of the yellow feeder guard. The bolt came loose & his left forearm hit a piece of threaded rod. Bruised arm and ice pack applied. Mounting/guide pins have been fabricated by the T&D shop to replace the threaded rods.

  6. Jun 07 – Jun 13 Longview June 12, 2014 Employee was grinding and welding in weld shop. Left room and upon return found box of metal parts on floor on fire. Fire extinguisher used to put out fire. Parts should have been put away immediately and container properly disposed of. Removed flammable material from weld shop.

  7. Jun 07 – Jun 13 Winston-Salem June 12, 2014 EE was returning a wood plank that was used for Washer support during maintenance to the normal storage rack area on the washer mezzanine when a splinter of wood penetrated his cut resistant glove. Investigating to locate a heavy duty plastic style plank to use.
