INTRODUCTION • BAIC, Ltd. is a multidiscipline company that provides services to the Petrochemical and Refining Industry. • Our service includes: Construction, Maintenance, Turnarounds and Capital projects. • Our divisions include: Demolition, Investment Recovery, Industrial and Environmental work activities. • We have been in business for over 8 years with the primary focus on the Gulf Coast • We have also provided services to Trinidad, Aruba, Central and the East Coast of the U.S. • All with an achievement of one recordable in eight years.

SAFETY CULTURE • Safety is not considered a priority that can be shifted depending on the demands of the situation, rather a value linked with every task. • We believe total employee involvement in the planning and execution process is essential to achieving the highest level of safety performance. • Which in return has helped us developed one of our greatest attributes in 2008, a tailored JSA designed for each craft.

  5. BAIC JSA DOCUMENT Task Planning Process • Job Safety Analysis (JSA) (JHA) (SPA) (est.) • Segregated and itemized one issue at a time. • Confirmed systems were being utilized. • Additional reminders / end of shift check list. • Comments and concerns were available to be expressed.

  6. FIRST ISSUE ADDRESSED Not verifying that the work crew acknowledged the following: • Client specified safety requirements or forms. • Understanding permit requirements and additional preparations prior to work.

  8. SECOND ISSUE ADDRESSED Ensuring employees knew & understand critical hazards such as: • Fall protection plan • Confined Space Entry • Lock Out Tag Out Process • Concerning issues / Hold points

TRADITIONAL JSA PROBLEMS Traditional Job Hazard Analyses were designed with three main things in mind: • What task are you about to perform? • What tools & equipment will be used during the process? • What mitigation or elimination steps will be taken into consideration to resolve each hazard as the task is executed? Problems: • Employees were expected to write a narrative explaining a step by step process. • Additional tools and equipment. • Additional task or change in task. What does this create? • Confusion for field auditors, new employees to the crew, est.

  13. ADDITIONAL MODIFICATIONS Supplementary add-ons: • Housekeeping taken into consideration. • Confirming our buddy system was being utilized. • Establishing a last minute / end of shift check list. • Ensuring incident reporting. • Comment and concerns are available to be expressed.
