Submitting an Injury Report to OHR-WC • The following slides describe how to submit an injury report to OHR-WC. • All safety officers and campus investigators will have access to the review and approval site for their area. • Additional individuals may be added to the approval site as alternates. Please notify OHR-WC if you would like to make changes to who may approve a form in your area. Workers' Compensation, Office of Human Resources

  3. Email Notification • You will receive notification by email that a report has been completed for an employee. • Select the link to review the form. Workers' Compensation, Office of Human Resources

  4. Enter Your User ID • Notify OHR-WC if you should have access to the site but are unable to log on. Workers' Compensation, Office of Human Resources

  5. If the form has already been submitted to OHR-WC your will receive this notification. • You may view the form by selecting the link. Workers' Compensation, Office of Human Resources

  6. Review the Form • If the form has not already been submitted to OHR-WC you will be connected to it. • Please review the form and make any necessary changes/updates. Workers' Compensation, Office of Human Resources

  7. Add Comments • A text box has been added to the form to allow for Safety Officer comments. • Safety Officers are encouraged to share information gathered during their injury investigation in this space. Workers' Compensation, Office of Human Resources

  8. Submit the Form • Please submit the form as soon as possible. • You do not need to complete your investigation of the claim before submitting it to OHR-WC; this is only a first report. • Once submitted you may not make changes to the form. Workers' Compensation, Office of Human Resources

Claims History Access • You may access printable versions of all forms your area has submitted through the following link: https://workserv.ohr.psu.edu/WorkersComp/ SafetyOfficer/logon.cfm • This site lets you view your area’s claims history list as well as claims that still need to be reviewed.   10. Claims History • Select “List claim history” to view all reports submitted by your area. • Select “View” to access the claim information. • This list is organized by the employees’ names. Workers' Compensation, Office of Human Resources

  11. Claims to Review • Select “List claims to review” to access the reports that are still awaiting your approval. • Select “Review” to review the form. • This list is organized by the date you received the form. Workers' Compensation, Office of Human Resources
