New User Registration Page When prompted, enter Invitation Code

Returning User Log In Page Enter username and password

Returning User Confirmation Confirm previous address and phone number, check Consent to Test and check T&C’s

Registration Confirmation Email Both New Users and Returning Users will receive a registration confirmation email

Select Your Screening Option Click on the “Select an On-Site Screening” button

State Selection Choose state from dropdown menu for preferred location to participate

City Selection Choose city from available list for preferred location to participate

Appointment Selection Choose appointment time from dropdown list of available times on preferred clinic date

Register for Appointment Click “Register”

Registration Complete Appointment is registered Appointment can be changed or canceled at any time

On-Site Appointment Confirmation Email Confirmation email sent immediately upon registration Reminder emails will be sent 1 week and 1 day prior to the scheduled appointment

Health Dashboard Participant can log in to view appointment details, edit or cancel at anytime
