  7. 1. Resume & Certifications • A current resume • Copies of certifications • Supplemental Artifacts: • Cover letters • List of references • Background information on teacher and teaching context • Educational philosophy and teaching goals

  8. 2. Student Growth Objectives (SGO) • List of assessment/s used to measure SGO • SGO Goals • Deadlines established to meet SGO goals • Supplemental Artifacts: • Copies of assessment tools

  9. 3. Evaluations & PIP • Evaluation Reports for the current school year • A copy of the previous year’s PIP for everyone not new to the district • Supplemental Artifacts: • Any informal observations recorded

Professional Development Documents • Certificates received for professional development you have attended for the cycle • List of professional development workshops conducted by you • List of the current school year’s professional development hours • Supplemental Artifacts: • Information you received at the workshops you attended • Notes you took at a workshop • Copy of handouts given by you at your workshops

Samples of Student Work • Complete samples of the writing process for written assignments • Mathematical or scientific open ended problems showing the student’s work • Finished products or pictures of student presentations • Rubrics • Supplemental Artifacts: • Video links to student presentations • Pictures of models or posters created by the students.
