  6. Introduction letter and questionnaire June 2007 • All practices in Central Norfolk • 32 responses. • Analysis and presented to EOL group. • Specific areas and then important points.

  8. Does your practice maintain a list of patients undergoing palliative/terminal care? Is this list prominently displayed? • Are palliative care patients discussed at a meeting with the PHCT present to agree advanced planning? How often are these meetings held?
