Student pharmacists from the University of the Pacific help can you! • What can we do for you? • Help review your Medicare Part D Plan • Potentially save you money on your Part D medications • Review all of your prescription medications to make sure they are safe • Give you a seasonal flu, pneumococcal or shingles vaccine • Check your blood pressure, cholesterol, bone density, blood sugar levels • and much more • Remember to bring: • Your red, white and blue Medicare card • All of your prescription medications Date: Saturday, November 12th Time: 11:15am-6pm Location: Seven Trees Community Center 3590 Cas Drive, San Jose, CA *Appointments are preferred and encouraged.
