  2. Immunization of School Pupils Act R.S.O. 1990, CHAPTER I.1 3 (1)  The parent of a pupil shall cause the pupil to complete the prescribed program of immunization in relation to each of the designated diseases.

  3. Immunization of School Pupils Act R.S.O. 1990, CHAPTER I.1 11(1) Every medical officer of health shall maintain a record of immunization in the form and containing the information prescribed by the regulations in respect of each pupil attending school in the area served by the medical officer of health. "school" means a "private school" and a "school" as defined in the Education Act and includes a kindergarten, a junior kindergarten and a beginners class within the meaning of the Education Act.

  4. Immunization of School Pupils Act R.R.O. 1990, REGULATION 645 Amended to O. Reg. 443/03 1.A record of immunization maintained by a medical officer of health with respect to a pupil shall contain, (a) the name of the pupil in full; (b) the date of birth of the pupil; (c) the sex of the pupil; (d) the name of the schoolattended by the pupil; (d.1) the pupil's health number, as defined in the Health Cards and Numbers Control Act, 1991 ; (e) a record of all the pupil's immunization against designated diseases showing, (i) the type of vaccine given, (ii) the date of administration of the vaccine, and (iii) any reactions to the vaccine; (f) any statement of medical exemption that pertains to the pupil showing the effective time period on the statement; and (g) any statement of religious belief that pertains to the pupil.

  5. Education Act R.S.O. 1990, CHAPTER E.2 266. (2.1) The principal of a school shall, upon request by the medical officer of health serving the area in which the school is located, give that medical officer of health the following information in respect of pupils enrolled in the school: 1. The pupil's name, address and telephone number. 2. The pupil's birthdate. 3. The name, address and telephone number of the pupil's parent or guardian.

Education Act R.S.O. 1990, CHAPTER E.2 "school" means, (a) the body of elementary school pupils or secondary school pupils that is organized as a unit for educational purposes under the jurisdiction of the appropriate board, or (b) the body of pupils enrolled in any of the elementary or secondary school courses of study in an educational institution operated by the Government of Ontario, and includes the teachers and other staff members associated with the unit or institution and the lands and premises used in connection with the unit or institution.
