LOSS PREVENTION AUDIT • AGENCIES THAT DEVELOP, IMPLEMENT AND ADHERE TO A COMPREHENSIVE SAFETY AND LOSS PREVENTION PROGRAM WILL BE GIVEN A CREDIT TO BE APPLIED TO THE AGENCY’S ANNUAL SELF-INSURED PREMIUM PER LINE OF INSURANCE COVERAGE EQUAL TO FIVE PERCENT OF THE AGENCY’S TOTAL ANNUAL SELF-INSURED PREMIUM. COVERAGE FOR MEDICAL MALPRACTICE AND ROAD HAZARDS SHALL BE EXCLUDED. ACT 11 GOVERNOR M.J. FOSTER APRIL 9, 1998

LOSS PREVENTION AUDIT AN AGENCY WHICH HAS FAILED TO RECEIVE CERTIFICATION AFTER UNDERGOING A LOSS PREVENTION AUDIT SHALL BE LIABLE FOR A PENALTY OF FIVE PERCENT OF THE AGENCY’S TOTAL ANNUAL SELF INSURED PREMIUM PAID PER LINE OF INSURANCE. LOSS PREVENTIONSAFETY TRAINING • Courses conducted throughout the State • Can conduct individual agency classes onsite: • At least guaranteed 15 employees will attend • Topic must be selected from LP Safety Training Course List • Request must be approved by State LP Supervisor over training or LP Manager • Classes posted on our web site

  26. Choose Available Reports from the Reports option on the menu bar.

  27. Select Agency Rep from the Report Groups drop down menu and click on the Filter button.

  28. The resulting list will show the reports that you can run.

  29. Choose the report you want to run from the list and click on the running man.

  30. The next screen will show if the report was successful. Click on the Report Status option button on the left navigation bar.
