Background: Walsall Area Partnerships • Walsall Local Strategic Partnership: new model of neighbourhood management and engagement: • six area partnerships identified each with dedicated area manager • increased levels of partnership engagement • direct reporting and action processes – ‘working smarter’ / cut out bureaucracy • ‘four level model’ • emphasis on community derived priorities • development of area plans, prioritising actions for communities e.g. ASB, litter, green spaces

Background: Walsall Area Partnerships • Health improvement not seen as a priority by the community • No holistic set of health outcome information at Area Partnership level • Opportunity to identify community level health needs/priorities and to provide guidance on appropriate interventions • Public Health Area Partnership reps

  5. Area Partnerships: development of health profiles • A. Consultation • stakeholders: wide involvement in developing approach to profiles • indicators of health: what measures of health can/should we use? • geographical basis: what community areas can we provide meaningful information at?

Area Partnerships: development of health profiles • Examples of indicators • Basic demographics – population size, age profile, ethnicity, deprivation profile • Long term conditions prevalence • Average age at death in years by cause • Infant mortality rate • Proportion of obese children • Chlamydia screening • Accident and emergency self referrals rate per 1,000 pop • ACORN segmentation profile to inform marketing / social media campaigns in the areas

Prioritisation • Pick out key health issues for the Area Partnership as a whole • Identify community level inequalities within Area Partnerships • Priority matrix for quick review of issues • C. Key interventions • For each priority issue what are the key services or interventions that are available or would be suitable for the communities in the AP?

Health profiling local areas: the challenges • Time and resource • Agreeing on health indicators - tension between using current strategic priorities as indicators or letting the indicators drive the priorities • Data collection and collation – not everything we wanted was available or accessible • Interpretation of data for partners

  12. Impact • Local Area Plans • Robust health input to Area Plan priorities and action plans • enabled inequality-based planning • baseline/context information for providers of local services • targeting of services into areas of greatest need

Case Studies • Littering / traffic congestion identified as priorities by communities – PH tied this to obesity agenda/ reduced activity levels- walking buses / healthy food mapping / planning • Links made with SERCO, Walsall Housing Group to deliver NHS Health Checks in community identified priority areas. • Out reach sexual health / community engagement with youth services and established providers to identify right place right time.
