Injury Mishap Report • Outside employees not currently required to report • Goal: Class A - 0, Class B - 0, Class C - <13, Class D - < 14 • Mishap report criteria • Class A: Fatality or Total Disability • Class B: Partial Disability • Class C: LWD > 30 Days • Class D: All Others • Class E: Near Miss

  6. Property Damage Mishap Report • Goal: Class A - 0, Class B – 0 Class C - < 5, Class D < 268 • Mishap report criteria • Class A: Damage/Loss > $1,000,000 • Class B: Damage/Loss > $ 200,000 • Class C: Damage/Loss > $ 20,000 • Class D: All Others • Class E: Near Miss
