  2. RPMs missing on lane lines - Index 600, Sheet 13 requires RPMs to be installed on all lane lines at 5 feet center to center in the approach and transition of the lane shift.

  3. Temporary Lane Separator has white delineators - Index 600, Sheet 11 requires orange tubular markers or orange and white vertical panels.

  4. White temporary lane separator was installed - Index 600, Sheet 11 requires the temporary lane separator to be yellow.

  5. Pedestrian signal pole is placed in plastic drum encased by milling material - Spec 102-9.1 requires all temporary traffic control devices to be a QPL product which ensures crashworthiness.

  6. Drop-off is greater than 3 inches located within the clear zone behind curb and gutter [Index 600, Sheet 10 requires a barrier for drop-offs greater than 3 inches within the clear zone or 1:4 shoulder treatment].

  7. Mixing of devices at driveway - Index 600, Sheet 9 prohibits intermixing of devices.

  8. Use of non-standard pedestrian channelization on bridge [Index 600, Sheet 2 requires use of pedestrian channelizing device listed on the QPL].

  9. Need advanced Signs.

  10. Above ground hazard located in clear zone [Index 600, Sheet 3 requires a 14 clear zone for 35 mph [posted speed limit].

  11. Business entrance signs were on portable sign stands [Index 600, Sheet 6 requires all signs to be posted when work operation exceeds one day]. • Clear Sight distance not adequate. [Index 600, Sheet 2 requires adequate clear sight distance.

  12. The third cartridge in the Quadguard crash cushion was a Type II - Per the QPL Vendor Drawings, it should be a Type I.

  13. Crash cushion cartridge is damaged/crushed - Replace damaged cartridges per manufacturer’s requirements.

  14. Crash cushion nose support missing - Nose support is required per the manufacturer’s installation manual.

  15. Loose nuts on anchor bolts for crash cushions installed on concrete - QPL Vendor Drawing require 120 ft-lbs.

  16. Debris/above ground hazard in clear zone - Index 600, Sheet 3 requires 30 foot clear zone (runout). The stockpiled debris is between the departure line and the clear zone which shall be free of hazard.

  17. Barrier and crash cushion are not offset which causes the transition panel to be attached improperly - Index 415, Sheet 9 requires the barrier and crash cushion to be aligned and slightly offset when a transition panel is installed.

  18. Crash cushion foundation thickness questionable on shoulder - QPL Vendor Drawings require minimum of 6” of asphalt.

  19. Transition from guardrail to temporary barrier wall is not standard - Index 414, Sheets 9, 10, and 13 provides standard attachment to bridge rail – Type K barrier is required.

  20. Barrier wall on bridge does not have transition pattern - Index 414, Sheet 8 requires transition units anchor pattern of 3, 3, 2, 1.

  21. Barrier wall was not transitioned from freestanding to staked – Arrows below represent the transition pattern - Index 414, Sheet 8 requires transition units anchoring pattern of 1, 2, 3, 3, staked/anchored.   22. Nonstandard connection to bridge railing -Index 414, Sheet 9 requires transition splice to connect Type K to bridge railing.

  24. Deflection space behind low profile barrier has stored material and other debris - Index 412, Sheet 2 requires 9 inches deflection space.

  26. Type K Barrier is located within the deflection space of the low profile barrier wall - Index 412, Sheet 2 requires nine inches of deflection space.

  27. Correct • White delineators are used on low profile barrier and the object marker is missing [Index 600, Sheet 12 requires orange tubular markers with white sheeting. Index 412, Sheet 1 requires a Type I object marker placed at end of barrier].

  28. Type K abutting Low Profile Barrier - The Design Standards do not provide a connection detail for dissimilar barriers that deflect different amounts.

  29. Old anchor plate bolts are on traffic side of barrier - Cut off to prevent vehicle from snagging.

  30. Less than two foot of paved deflection behind barrier wall on a 1:10 slope - Index 414, Sheet 6 requires 2 feet of paved deflection space on a 1:10 slope for 45 mph or greater.

  31. Missing anchor plate - Index 415, Sheet 4 requires two anchor plates on barrier abutting crash cushion.

  32. Stored material is located within the deflection space - Index 415, Sheet 1 requires two feet of deflection space for 45 mph speed or less.

  33. Overlap of temporary barrier and no clear space between walls - Index 415, Sheet 6 requires a minimum of six sections of barrier wall at overlap and two feet of clear space between the walls.

  34. Type K Barrier has partial fill in the deflection space - Index 414, Sheet 7 requires a minimum of 5’ wide back fill for the full height of the barrier.

  35. Barrier wall staked on back side - Index 414, Sheet 6 requires barrier to be staked on the traffic side.

  36. Type K Barrier wall was staked in median - Index 414, Sheet 7 requires barrier to be freestanding in the median.

  37. Use of Index 415 barrier wall connecting pins for Type K [Index 414, Sheet 4 requires a rectangular top plate connecting pin – pin used for 415 barrier is weakened due to snake pin hole which is located at shear point and cannot be substituted]

  38. Barrier wall connection loops missing and broken - Replace barrier. Specs. 102-9.1 requires that all traffic control devices meet acceptable standards as outlined in American Traffic Safety Services Association (ATSSA’s) “Quality Guidelines for Temporary Traffic Control Devices and Features”.

  39. Drainage slots are filled with milling material - Spec 102 requires traffic control devices to be maintained.

  40. Overlap of temporary barrier and no clear space between walls - Index 415, Sheet 6 requires a minimum of six sections of barrier wall at overlap and two feet of clear space between the walls.

  41. Barrier placed on non flat surface - Index 415, Sheet 1 requires 1:10 or flatter surface.

  42. Transition from guardrail to temporary barrier wall is not standard - Index 414, Sheets 9, 10, and 13 provides standard attachment to bridge rail – Type K barrier is required.

  43. Barrier installed on top of curb – blunt end in clear zone [Index 415, Sheet 1 requires 2 feet of paved deflection space on a 1:10 slope for 45 mph or less].

  44. Barrier is installed backwards [Index 414, Sheet 4 provides the details of connection between barrier units].

  45. No transition from freestanding to backfilled barrier [Index 414, Sheet 8 requires staked transition of 1, 2, 3, 3 in advance of backfilled barrier].
