  3. Who Needs a PPP? Has the student been identified as requiring intensive supports? Yes No Does the student have learning outcomes that differ substantially from provincial curricula? Develop a PPP Yes No Can the needs be met through Differentiation? Does the student require individualized support? Yes No Yes PPP not required Develop an ROA

  4. Impact Assessment Form • 3 or more checkmarks in areas of need • Support in all three areas - reflection within PPP.

  6. IS Checklist – To remember • Ministry ID – 9 numbers • Name – include alias’ • Assessments – new • Support matches the areas of impact. • Outside Agency – CBO’s

  7. IS Checklist – To remember • Checked areas need to coincide with PPP goals. • Ex: if you have “Personal/Social Well-being” check off as an area of impact, then a goal needs to be developed for that area.

  8. IS Checklist – To remember • Support matches the areas of impact. • Ex: if the area of impact is in “Motor skills/Sensory” support should come from an OT from the School Division Team.

  9. IS Checklist – To remember • Delineate support from EA and SST • Look very carefully at the Frequency of support by the School, School Division and Outside Agency Teams – this should be reflected in the PPP.

  10. PPP Checklist – To remember • Note strengths – not just a school perspective – home/community • Current Level of Performance – assessment • Goals/Outcomes = Areas of Need on Impact Assessment • By May 2010, Tina will independently …. In 4 out of 5 consecutive trials.
