8. Libraries Why use these? (don’t we have Google?) Libraries: Google = only 7% available health information: libraries have databases, ejournals, ebooks, networks of contacts for free at point of use resources Librarians/information specialists - trained to a high level of information retrieval, organisation and knowledge, make contacts, select resources Best research evidence needed for evidence based practice

Evidence based practice: a systematic approach to clinical problem solving which allows the integration of the best available research evidence with clinical expertise and patient values.(Sackett DL, Strauss SE, Richardson WS,et al. Evidence-based medicine: how to practice and teach EBM. London: Churchill-Livingstone, 2000) In NHS Constitution under Patient Rights: ‘You have the right to expect local decisions on funding of other drugs and treatments to be made rationally following a proper consideration of the evidence’

Evidence based practice: why? systematic approach - justifiable best available research evidence – for the most reliable treatment clinical expertise – puts research evidence into context with experience patient values – patients have to agree to treatment before they receive it promised in NHS Constitution therefore expected google – only 7% of available health information but everyone (incl patients) goes there – have something better!

Dynamed Plus • Evidence-based summaries • Over 3,200 topics written and reviewed by clinicians • Most current of the point-of-care tools (BMJ, 2011) • Mobile app for Android or iPhone/iPad • Epidemiology, diagnosis, management, treatment, guidelines and prognosis • References and grading of evidence to support evidence-based practice • Simple to search across general search, medical calculators, patient info, recent updates, specialties • Includes drug information from BNF

33. Searching the databases: PICO P = Patient, population or problem I = Intervention (treatment), prognostic factor or exposure C = Comparison O = Outcome(s) of interest PICO is a guideline to help you structure a search. You do not have to use all the sections.

Searching using subject headings Each record in the database is indexed under several subject headings Searching using subject headings will include words with different spellings (eg anesthetics/ anaesthetics) and synonyms (eg postnatal/postpartum) Searching using subject headings gives a wider coverage of the subject

Searching using free text Free text is where you just type in the words Best used for specific things to search for To search for a phrase use “ …” eg “Loewenstein Occupational Therapy Cognitive Assessment” To search for variations on a word use * eg. surg* will find ‘surgery’, ‘surgeon’, ‘surgeries’etc. Do not have to tick ‘Map to thesaurus’ – just click on ‘Search’ Other processes are exactly the same as with the thesaurus/subject headings

37. Searching your chosen database Use one database at a time Remember to put in search terms separately Each search is a separate search until you combine Combine with AND or OR

38. Combining with AND or OR Or is MORE • Increases the number of results • Use for two terms for the same thing or two similar concepts AND: • Reduces the number of results • Combines two different terms or concepts

39. Combining with AND • AND retrieves records in which all your search terms appear Heart attack Viral wheeze Aspirin Viral wheeze AND Reduces the number of records Retrieves articles where both the words ‘heart attack’ and ‘aspirin’ appear, where both search terms appear

40. Combining with OR OR broadens your search Heart attack OR Myocardial infarction Finds records containing either the words ‘myocardial infarction’ or ‘heart attack’ Increases number of results. OR is MORE!

41. Combining with NOT NOT removes all results with the term you do not want in it Aspirin Anti-platelet therapy NOT Reduces number of results.
