  4. SEPT - MANDATORY TRAINING Health & Safety - Reporting • DATIX Form Completion • DATIX Forms are very easy to complete. One should be completed immediately after an incident or accident happens and anyone can complete the report form. A username or password is not required to complete and submit a DATIX form via the intra-net link (this is only required by Managers who need to access the form after it has been submitted). • Witness Statements, and other relevant documents, should be scanned and attached to the DATIX form. • Once a DATIX form has been submitted the system automatically sends out any necessary alerts; the form is then available for the manager in the area identified to complete their part, including any investigations or reports. COURSE OBJECTIVES On completion of the Health & Safety Reporting section, you will understand: • The reporting process • RIDDOR reports Accident & Incident Reporting Within SEPT accidents and incidents are reported electronically on a database system called DATIX. DATIX can be accessed via a link on the front page of the Trust intra-net site; the link is located in the menu down the left side of the page. All staff have a duty to report an incident or accident when it happens, but only one report is required per incident.

SEPT - MANDATORY TRAINING RIDDOR • Certain accidents or incidents may need to be reported in a particular way. • These are termed as RIDDOR, which stands for: • The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations. • This is an Act of Parliament which the Trust must comply with. • RIDDOR Includes • Things which must be reported under RIDDOR include: • Any fracture, not including fingers, thumbs, or toes • Any amputation • Dislocation of the shoulder, hip, knee, or spine • Loss of sight (temporary or permanent) • Chemical or hot metal burn to the eye • Any penetrating injury to the eye • Any injury resulting from electric shock or electrical burn • RIDDOR Incidents • A reportable RIDDOR incident also includes time-off sick after an accident/incident at work, where the person is off sick for more than three days. • Employees who are off-sick because of an incident/accident whilst at work must, by law, inform their manager once they are off-sick for more than three days. The Manager then reports this fact to the RMD. • The RMD (Risk Management Department) is based at Pride House in Laindon. • All RIDDOR incidents must be notified both on a DATIX form and by telephoning the RMD on the number below. • Telephone Number: 01268-407-811

  6. SEPT - MANDATORY TRAINING ReportingInformation All reports should be made to the Risk Management Department (RMD). This Department manages a variety of matters within the Trust, including Health & Safety. The RMD is based at Pride House in Laindon. Telephone: 01268-407-811 E-mail: Brian Dean, Irenaeus, and Andy Hamilton Safe-haven Fax: 01268-407-810 RIDDOR Reporting RIDDOR reports are made in the usual way on a DATIX Form and the RMD should be notified immediately by telephone on 01268-407-811. Updates and changes to the DATIX Form should be made by the manager or their deputy as they happen. It is very important that staff report any possible RIDDORs to the RMD as soon as possible. The RMD has to report the incident to the HSE within 10 days of the incident happening. This includes weekends and bank holidays. (The 10 day period is set by law, and we cannot change it. If the Trust fails to comply both the persons involved and the Trust can be prosecuted for this failure under the Regulations.)
