  3. PHYSIOPATHOLOGIE • DISTENSION VESICULAIRE • Obstruction du sphincter d’ODDI • Morphinomimétiques • Absence de contraction vésiculaire • Jeûne • Stase biliaire et Sludge • Jeûne prolongé • Précipitation de sels biliaires et de cholestérol (favorisée par l’hyperbilirubinémie)

Curr Opin Crit Care. 2003; 9(2): 127-32. • Vascularisation vésiculaire fragile (Bas débit splanchnique) • INFECTION • Systémique • Injection endotoxine E.ColiCAA chez l’animal • PAF injecté dans l’artère cystiqueCAA histologique • Locale • Stase biliaire et pullulation microbienne gastro-duodénale

  5. PHYSIOPATHOLOGIE Dans les modèles expérimentaux, il faut associer au moins 2 des 3 facteurs précédents pour obtenir une cholécystite alithiasique

  8. CHOLECYSTITE AIGUË SUPPURREE La muqueuse est largement ulcérée Infiltration inflammatoire massive de la sous-muqueuse

  9. ANATOMO-PATHOLOGIE L’absence de lésion muqueuse élimine le diagnostic de cholécystite aiguë

JAMA. 2003; 289(1): 80-6. • LOCAUX • Douleur hypochondre droit spontanée ou provoquée (Murphy) (40 à 100%) • Défense • Ictère • GENERAUX • Température (50 à 100%) • Sepsis Une cholécystite aiguë alithiasique doit être recherchée chez tout malade de réanimation présentant un sepsis inexpliqué.

  12. SIGNES BIOLOGIQUES AUCUNE SPECIFICITE • Hyperleucocytose (50 à 100%) • Hyperbilirubinémie • Cholestase

Anesthesiology 1986; 65:A 78. • Epaississement pariétal vésiculaire > 4 mm (aspect feuilleté de la paroi) • Diamètre transverse de la vésicule > 4 cm • Grand axe vésiculaire > 10 cm • Epanchement liquidien périvésiculaire • Sludge et/ou lithiases vésiculaires • Complications: empyème vésiculaire, rupture, hémorragie… Une vésicule échographiquement normale élimine le diagnostic avec un quasi certitude.

  14. CAUSES D’ EPAISSISSEMENT DE LA PAROI VESICULAIRE • Cholécystite aiguë • Cholécystite chronique • Ascite • Hypertension portale • Hypo-albuminémie • Insuffisance cardiaque • Insuffisance rénale

SIGNES TOMODENSITOMETRIQUES(Présentation non univoque ou suspicion de complication) • Signes superposables à ceux de l’échographie • Ruptures vésiculaires • Cholécystite hémorragique: contenu hyperdense • Cholécystites gangréneuses/emphysémateuses • Hypodensités liquidiennes ou gazeuses • Epaississement pariétal pseudotumoral • Urgence chirurgicale • Abcès périvésiculaire

  21. TRAITEMENT • CHOLECYSTECTOMIE ( Référence ) • Affirme le diagnostic • Recherche une complication ( perforation ) • Elimine une autre cause de sepsis abdominal LAPAROTOMIE  CŒLIOSCOPIE

  22. TRAITEMENT • CHOLECYSTOSTOMIE PERCUTANEE* *GLENN F. Cholecystostomy in the high risk patient with biliary tract disease. Ann Surg 1977; 185:185-91. • Patients inopérables • Voie trans-hépatique le plus souvent • Repérage échographique • Drain dans la vésicule • Peut se faire au lit du malade

TRAITEMENT • CHOLECYSTOSTOMIE PERCUTANEE • Confirme le diagnostic si • Bile hémorragique ou purulente • Amélioration clinique • Cholécystographie pour apprécier la position du drain + cholangiographie (VBP) • Complications • Péritonite biliaire • Perforation organe creux • Hématome hépatique • Hématome local

  24. TRAITEMENT • CHOLECYSTOSTOMIE PERCUTANEE • Durée du drainage: 10 jours à plusieurs semaines • Cholécystectomie secondaire éventuelle (10%) de manière beaucoup moins systématique qu’au cours de la cholécystite lithiasique. • Traitement médical: antibiothérapie à large spectre.

  26. CHOLECYSTOSTOMIE PERCUTANEE TRANS-HEPATIQUESPIRA RM. Percutaneous transhepatic cholecystostomy and delayed laparoscopic cholecystectomy in critically ill patients with acute calculus cholecystitis. Am J Surg 2002; 183: 62-6. • 55 patients sur une période de 5 ans. • 23 en choc septique ou sepsis sévère (42%). • CPTH réalisée avec succès chez 54 patients / 55 (98%). - 1 hématome hépatique (laparotomie) Complications - 9 déplacements de cathéters - 3 décès par MOF • Amélioration clinique rapide (59% à 24h – 96% à 72h). • Durée moyenne d’hospitalisation: 15,5 +/- 11,4 jours. • 44 patients de retour à domicile, cathéter en place. • 30 patients opérés à distance (2 laparotomies – 28 cœlioscopies dont 4 conversions) • CPTH sûre et efficace chez des patients fragiles. (ASA 4: 39% - ASA 3: 34,5% - ASA 2: 34,5% - ASA 1: 2%)
