  2. FEI – AZ Academic Awards Application Application Instructions: The completed application and required attachments should be turned in to BA 223Q ATTN: Scholarship Committee, by March 5, 2012. The required attachments are as follows: 1) Completed Application 2) Current resume 3) Letter of recommendation from university professor 4) Essay that describes your background and how it has influenced your career ambitions in 500 words or less 5) Completed curriculum that lists each course taken and the final grade for each course. Please be sure to display cumulative GPA and GPA in major to date, as well.

FEI – AZ Academic Awards Application University: Applicant Name: Major: School: Address: Email address: Contact phone number:  Minimum Requirements: Declared undergraduate or graduate major of accounting or finance Letter of recommendation from a university professor GPA of 3.0 overall and 3.0+ in major Professional and civic involvement  PLEASE ENSURE ALL FIELDS ARE COMPLETED AND ALL ATTACHMENTS ARE INCLUDED. AN APPLICATION WILL BE REJECTED IF THESE REQUIREMENTS ARE NOT MET.
