  1. CASO CLÍNICO-3 GIST c-kit (-) Xavier García del Muro Solans Institut Català d’Oncologia. Barcelona Febrero – 2007

  2. CASO CLINICO: • Varón de 55 años, sin antecedentes de interés • Ingreso de Urgencia en Febrero-05, por cuadro de HDA. La gastroscopia mostró masa en fundus y cuerpo gástrico vegetante y ulcerada • El TAC mostró una gran masa en epigastrio, metástasis hepáticas y voluminosa carcinomatosis peritoneal • Biopsia: Carcinoma indiferenciado. IHQ: CAM 5.6 +, CAM 5.2 -, EMA -, C-kit -, CD 34 -

  3. Debido a un primer informe oral, sin datos de IHQ, se inició Imatinib 400 mg/día vo, ya que existía sangado digestivo persistente PreTratamiento

  4. En la visita 1 mes más tarde, al disponer de APª definitiva, se apreció mejoría clínica importante, y un TAC mostró reducción del Tm • El estudio mutacional mostró mutación de KIT a nivel de exon 11. Fue clasificado como GIST c-kit (-) Tras 6 meses de Tto

  5. A los 14 meses: progresión a nivel de masas peritoneales • Tto posterior con Imatinib 800 mg/día vo, ensayo clínico de Imatinib asociado a Doxorubicina, sin respuesta. Actualmente en curso de Sunitinib
