  5. INTRODUCCIÓN

Situación Nacional, regional y local o institucional • La falta de eficacia de la RPPI puede atribuirse, en parte, a la ausencia de indicaciones y expectativas uniformes de tratamiento; debe existir una razón justificada para llevar a cabo la RPPI, junto coa un conocimiento íntegro de las indicaciones, contraindicaciones, técnica, riesgos, costo y eficacia.

SITUACIÓN ACTUAL • Las atelectasias postoperatorias aparecen como complicación pulmonar, secundarias a una disminución de la capacidad vital, bajos volúmenes corrientes, mala distensibilidad pulmonar e inadecuada higiene bronquial, ocasionando un decremento de la CFR, un cortocircuito intrapulmonar aumentado, un mayor desequilibrio en la relación ventilación perfusión, aumento de la diferencia alvéolo arterial y finalmente hipoxemia
