    2. www.healthandsafetytips.co.uk When Should Form F10 Be Sent to the HSE? The form F10 should be sent to the HSE as soon as the Planning Supervisor has been appointed to the project. If the project is for a domestic client, the form should be sent to the HSE as soon as the contractor has been appointed to the project.

Date of sending the form Address of the construction site Name and address of the client Type of project Name and address of the Planning Supervisor Acknowledgement by the Planning Supervisor that they have been appointed
