  2. Overview The Structured Problem Editor is a tool for managing the “Problem List”. It helps users enter new problems into the Problem List and facilitates the “encoding” of problems. “Encoded” problems facilitate automated decision support, demonstration of meaningful use of the Problem List, and the exchange of problems between multiple information systems. Click the Home Icon to return to the main menu.

  4. Icon When to use this Icon Confirm this entry is a problem. Return problem to “Unknown” status. Add this entry as a comment to the previous problem/entry listed above. Icons Click the Home Icon to return to the main menu.

  5. Add new problems • To add new problems to the Problem List: • Begin typing into the “Enter Problem Name” box at the bottom of the Structured Problem Editor. As you type, the Problem Editor will begin listing problems it believes that you might be searching. • Click on problem name from the list • Click the [Add Problem Items to List] button • Problem will be added to the patient’s Problem List as an “encoded”problem • If the problem does not appear in the list: • Select the [Add Problem Items to List] button • Problem will be added to the list exactly as typed • Problem will be saved as a “structured (but not yet encoded)”problem Click the Home Icon to return to the main menu.

  6. Modify existing problems • To modify existing problems within the Structured Problem list: • Select the existing problem in the list • Selected problem will be displayed in the Update Problem button • The current problem information will appear in the edit boxes below the Problem List (Enter Problem Name and Comments) • Click in edit boxes to update or modify the problem • Select [Update Problem] button to save the changes Click the Home Icon to return to the main menu.

  7. Delete problems • To delete an existing problem: • Click the X icon to the left of the problem. • The selected problem will be displayed in strike through font . This format indicates that this problem will be deleted . • Click the [Save] button to update changes. Click the Home Icon to return to the main menu.

  8. Save, Undo, & Refresh • Use the [Save] button to update ALL changes made (Add, Modify, Delete) • Use the [Undo All] button to undo any/all changes (Add, Modify, Delete) • Use the [Refresh] button to refresh the Problem Editor with the latest changes. *This includes any information that may have been saved by another user that happened to be modifying the Problem List at the same time . Click the Home Icon to return to the main menu.

  9. Add Comments Comments can be added when adding a new problem entry or when updating an existing problem Click the Home Icon to return to the main menu.

  10. View details for a problem • To view additional details about a problem: • Click the (i) ‘Information’ icon to the far right of the problem. • The Problem Details dialog will be displayed. • Various details about the respective problem will be available for review. Click the Home Icon to return to the main menu.

  11. View all problems • To view a comprehensive list of a patient’s active and inactive Coded Problem Editorproblems; • Click the Actions link at the top of the Problem Editor and select View all problems. • The Problem View (Active and Inactive) Problems will be displayed in a window below. • The Problem View (Active and Inactive) Problems lists all coded problems that have been entered on a patient. Click the Home Icon to return to the main menu.

  12. Submit Feedback • To submit comments, suggestions, complaints, other about the Coded Problem Editor: • Select the User Feedback link. • The User Feedback dialog will be displayed. • Select a Category for the feedback you are providing. • Enter your feedback into the Comment field. Click the Home Icon to return to the main menu.

  13. SNOMED CT • The Problem Editor uses the SNOMED CT (Controlled Terminology) as its standard/source for encoding problems. • SNOMED CT is the clinical terminology recommended by the Healthcare Information Technology Standards Panel (HITSP) as the standard for documenting problems and diagnoses Click the Home Icon to return to the main menu.
