  3. Care Home Partnership Board Chair: Jacqui Glenning Outcome: A sustainable and innovative care home market for Torbay that supports quality, person-centred care and inclusive environments for people, as part of an integrated health, care and support system.

Purpose • Strengthen partnerships, joint working and information sharing. •  Improve health and wellbeing outcomes for residents and staff. •  Develop a shared understanding of the challenges (current and future) faced by the Torbay care homes market and address these together with innovative solutions. •  Right care in the right place.  

  5. Adult Social Care Priorities for improvement • Embed community led support approach to adult social care • Volume of people under 65 placed in residential care and in receipt of formal packages of care • Extra Care Housing • Home Care Transformation • Investment in Care Home market change
