  3. Prior to attending an orientation… • Log into the portal http://connect.cuchicago.edu • Click the “Resource” tab • Type in “School Counseling Practicum” • Read the following powerpoints: • Tutorial – Applying for School Counseling Practicum • Tutorial – School Counseling Clinical Requirements

Beginning the Practicum Process • Complete required coursework and practicum application • Site Selection • Tape Requirement • “School Counseling Practicum and Internship Sites” on CUConnect forms repository • Letter of introduction, site requirements, site supervisor qualification form • Type 73 Exam • Liability Insurance

  7. Insurance • Liability Insurance • PRIOR to starting your practicum or internship, candidates MUST submit their certificate of insurance to the School Counseling Clinical Office. In order to clock any hours, the School Counseling Clinical Office MUST have an updated copy of the certificate. • ASCA members insurance • When current certificate of insurance expires, please update and submit a new copy immediately. You may not clock any hours while your certificate has lapsed.

  8. Continuing the Practicum Process • Register for class • Forms for the clinical office • Signed Practicum Application form • Completed Site Supervisor Qualification form and Site Supervisor’s most updated resume • Signed practicum/internship agreement form • Most up-to-date copy of Certificate of Insurance NOTE: You may only start clocking hours once the School Counseling Clinical Office has confirmed that all your paperwork is in and the Concordia semester has started.

  9. CPS Requirements • For students planning on a practicum and/or internship in the Chicago Public Schools system (not charter schools), this process must be completed a year in advance. • For more information, please go to the Choose Your Future website (www.chooseyourfuture.org) or the forms repository on CUConnect.

  10. Remember… • Any needed forms can be found on CUConnect under “school counseling” in the forms repository.
