Ongoing Committee Initiatives Educational guidance on patient referral to kidney transplantation • Targets Referring Providers • Default pathway for CKD/ESRD patients should be transplant referral • Preemptive transplant is the goal and can only be achieved with timely “early” referral • (GFR range = 25-30) • Transplant education must begin long before ESRD to be most effective • (Stage 3-4 CKD)

Ongoing Committee Initiatives Educational guidance on patient referral to kidney transplantation • Major feedback themes: • Incorporating KAS information (dialysis waiting time points) • Appropriate referral timing • Transplant system impacts • Incorporating KDIGO 2012 clinical practice guidelines • Non-adherence

Ongoing Committee Initiatives Kidney Paired Donation Pilot Program update • Update on status of pilot program • Efforts to expand participation in pilot • MAC examination of minority impact: • Requests for demographic information on donors and candidates/pairs in pilot program (age and ethnicity) • Waiting list at participating centers; geographic location
