To Qualify You Must Meet One Of The Following: • Working At Least 40 Hours/Month Or, • Worked At Least 240 Hours In The Last 6 Months Before Applying Or, • If You Aren’t Working, Meet A One-time Deductible Based On Income *If You Qualify, You Will Have To Pay A Sliding-Scale Premium Based On Your Household Income.
