  4. Housing Stability: 0 – Stable housing with no known risks of instability 1 - Currently in stable housing but with significant risks of housing disruption 2 – Frequent moves or very unstable housing 3 - Currently homeless.

  5. Independent Living Skills: 0 – No evidence 1 – Mild impairment, generally addressable with training or supervision 2 –Moderate impairment, generally addressable with in-home services or supports 3 – Profound impairment, unable to live independently

  6. Transportation: 0 – No Evidence 1 – Occasional unmet transportation needs 2 - Frequent transportation needs 3 - Requires daily transportation

  7. Job Functioning/Vocational: 0 – Has clear & feasible career plans 1 – Has career aspirations & some plans to meet them 2 –Has career aspirations but needs assistance planning 3 – No career plans or aspirations 0 – No evidence 1 – Mild problems with work 2 –Severe problems at work. 3 – May have recently loss a job, not currently working nor recently employed

  8. Parenting/Caregiver Roles: 0 –Not a parent or in other caregiving role and/or there is no evidence of functioning inappropriately in a caregiver role 1 – Occasionally experiences difficulties with this role 2 – Struggling with these responsibilities, or issues are currently interfering with their functioning in other life domains 3 – Currently unable to meet these responsibilities or these responsibilities make it impossible for them to function in other life domains

  9. Medication Adherence: 0 – Not currently on any meds OR no evidence of unwillingness to taking meds as prescribed without reminders 1 – Usually collaborates and will take prescribed meds routinely, but needs reminders to take them regularly 2 – Periodically unable or unwilling to collaborate or take meds as prescribed 3 – Does not collaborate and has refused to take prescribed meds during the past 30 day period

  10. Educational Attainment: 0 - No evidence of need 1 – Has set educational goals and is currently making progress towards achieving them 2 – Has set educational goals but is currently not making progress towards achieving them 3 – Has no educational goals and lack of educational attainment interferes with lifetime vocational functioning

  11. Financial Resources: 0 – No evidence 1 – Mild difficulties. Has financial resources to meet basic needs with careful budgeting. 2 –Moderate difficulties. Has financial difficulties that limit their ability to meet needs. 3 – Severe difficulties. Experiencing financial hardship that prevents them from living independently.

  12. SUPPORT NETWORK: 0 – No evidence 1 – Access to healthy support network, but relatively new 2 – Limited access to support network 3 – No support network identified
