LA SECRETARIA DE SALUD MUNICIPAL INVITA AL CONCURSO DEL BEBE SALUDABLE Categorías: Lactante: De 0 a 6 Meses Gateadores: Menores de 1 (un) año Caminadores: Entre 1 y 2 años Requisitos: • Poseer carné de crecimiento y desarrollo diligenciado • Carné de vacuna con esquema completo de acuerdo a la edad • Resumen historia clínica del bebe que demuestre el estado de salud • Alimentación: Acreditar lactancia materna exclusiva hasta los 6 meses • Tener peso y talla adecuado para la edad • No haber sufrido ninguna enfermedad prevalente de la infancia tales Como EDA, IRA (bronconeumonía, neumonía, etc.), DNT, PPI. Hospitalización. Para inscribir el bebe se deben de adjuntar los siguientes documentos: - Fotocopia cedula de los padres - Carné de crecimiento y desarrollo - Carné de Vacunas - Carné de E.P.S. - Fotocopia historia clínica - Carpeta bien decorada. PLAZO DE INSCRIPCION HASTA EL 8 DE AGOSTO DE 2011 EN LA SECRETARIA DE SALUD MUNICIPAL (ALCALDIA).
