Risk of Harm to Others & Risk management Arrangements • Developed in 2011 and ratified early 2012 • Across service implementation • Identified 2 levels-level 1 screening, level 2-SPJ • Timeframes identified for each and audit pathway introduced • Standards for both risk assessment and risk management plans were detailed including review dates

Practice • Avon and Wiltshire (adapted) risk assessment screening tool completed for all inpatient and community patients within set timeframe • Assessment formulation agreed and signed off by clinical team • SPJ format agreed, responsible clinicians identified and ‘clinical supervisor’ identified • Risk review meeting held prior to completion of SPJ chaired by RMO or Clinical psychologist • Risk management plan agreed by risk review meeting and completed risk assessment/management plan presented to CPA group for sign off and implementation
