WHAT SHOULD I EXPECT? WHAT IS REHABILITATION? Mental health/co-occurring rehabilitation emphasizes learning and skill-building. This educational process is done on Community Support and Recovery Pathways Teams. • As a result of the skill building approach, you will learn to do things independently; • No recovery plan will be done without you; “Nothing about you, without you”; • Many services will be delivered in your home or community settings; and • There will be an emphasis on individual decision making.  WHAT ARE THE DIFFERENCES BETWEEN CSP AND RP? • Individuals on CSP teams will have more needs and receive more hours of rehabilitation services overall; • RP Teams will specialize in less-intensive / step-down rehabilitation services; • Individuals can move between CSP and RP services over time, based on their needs; and • Both CSP & RP Teams will have peer recovery support specialists. WHAT ARE MY RIGHTS on the Community Support/Recovery Pathways Teams? • To be involved in your individualized recovery plan, making goals that you choose and deciding what you would like to learn; • To receive services in your community that incorporates your weekly schedule; • To raise objections when you disagree with your provider and have those issues addressed; and • If you have a complaint, please contact: • [agency representative name / phone number]
