  4. Core Component 1: Support • New Member/Newcomer meetings • Psychosocial Screening & Personal Support Plans • Decision Assistance/Question List Counseling (Open to Options™) • Group Support for people with cancer • Group Support for Family, Friends, & Caregivers

Core Component 1: Support Cont… • Groups/other activities for children/teens who have a family member with cancer or who have cancer themselves • Cancer-related bereavement support for all ages • Short-term cancer counseling for individuals and families (Optional) • Awareness of & access to individual/group support online

  6. Core Component 2 : Education • Lectures/workshops about: • cancer and its treatment • psychosocial issues • Skill-building programs • Workplace, school & • Faith-based education/support programs • Awareness of & access to educational materials and programs online

  7. Core Component 3 : Healthy Living Classes • Mind-body programs • Exercise & physical activity programs • Nutrition education & awareness activities • Expressive Arts • Health maintenance & awareness

  8. Core Component 4: Social Connections • Activities interaction, play, & humor (e.g. , Pot Lucks, Gilda’s Got Talent, Noogiefest) • Social activities for families facing cancer who have children and/or teens • Awareness of & access to social networking opportunities online

  9. Core Component 5: Linkage & Referral • Personalized assistance & referral to resources within CSC and in the broader community as needed • Resource library & computer access • Awareness of & access to online information about local/national resources

National Helpline • Provides emotional support counseling for cancer patients, survivors, loved ones, and caregivers • Provides navigation on practical resources available to you • English & Spanish counselors available • Translation service available for 200 additional languages • M-F 9am – 9pm ET • Live web chat also available

  11. www.MyLifeLine.org • Interactive Blogging & Updates • Let family and friends know how you’re doing. We’ll email them when you post an update. • Helping Calendar • Coordinate volunteers to help with meals, rides to treatment, errands, child care, pet care, and more. • Discussion Boards • Connect with other patients and caregivers in an online forum to support each other with information, opinions, stories, and inspiration. • Care Coordinators • Make life easier by setting up family and friends as “Care Coordinators” to help manage your site and organize volunteers. • Privacy Controls • Your cancer experience is personal. Ensure the right people have access to comment on your site and volunteer.

  12. What is it? • Research survey to identify and understand emotional and social needs unique to the cancer experience • Informs medical professionals and patient advocates about patient needs to improve quality of care • Confidential and secure

Who is it for? • Caregivers (loved ones who help patients) • Cancer Patients/Survivors • People facing any cancer, including: • Acute Myeloid Leukemia (AML) • Breast Cancer • Metastatic Breast Cancer • Chronic Lymphocytic Leukemia (CLL) • Chronic Myeloid Leukemia (CML) • Lung Cancer • Melanoma • Multiple Myeloma • Prostate Cancer • Stomach Cancer
