  2. Maternal, Infant, and Early Childhood Home Visiting Program(MIECHV) Funding from the U.S. Department of Health and Human Services, Health Resources and Services Administration Authorized by the Patient Protection and Affordable Care Act of 2010

  3. Federal HV Program Goals Strengthen and improve the programs and activities carried out under Title V Improve coordination of services for at-risk communities; and Identify and provide comprehensive services to improve outcomes for families who reside in at-risk communities.

  4. Quantifiable, measurable improvements for the populations participating in the program. Benchmark Areas Maternal and Newborn health Child injuries, abuse, neglect, or maltreatment, and reduction of emergency department visits; School Readiness and Achievement Crime or Domestic Violence; Family Economic Self-Sufficiency; and Coordination and referrals for other community resources and supports.

Formula Grant Competitive “Development” Grant DC HV Programs The Framework is a key strategy supporting the efforts of the Home Visitation Program and the vision for the District is: “All children and families will have access to a continuum of comprehensive, high-quality early childhood programs and services that promote child well-being and school readiness and ensure that all children are healthy, ready to learn and have safe passage through the early years”. 5

At-risk communities: Wards 5, 7, and 8 Wards with the Poorest Health Indicators : Home Visiting Formula Grant Improving health and development outcomes for at-risk children through evidence-based home visiting programs. Programs: Parents As Teachers (PAT) Home Instructions for Parents of Preschool Youngster (HIPPY) Contractors: Mary’s Center = PAT Family Place = HIPPY 8

  9. Home Visitation Data Collection and Reporting System Real time data can be collected during home visits; reduces paper-based forms. DOH owns the data and has access to the system and the data at any time. Reports for model developers Special Features Continuous Quality Improvement Reports can be generated off of any variable in the system. The system can be interfaced with other data systems for data transmission. Can accommodate multiply programs and vendors. Data system's capacity to report similar data and indicators from multiple funding streams. Performance Management 9

At-risk communities: All Wards High Risk Priority Population: • Low-income eligible families • Eligible families who are pregnant who have not attained age 21 • Eligible families that have a history of child abuse or neglect or have had interactions with child welfare services • Eligible families with children with developmental delays or disabilities.
