Sacramento County Primary Care Center (SCPCC) • Central Primary Care Clinic for Uninsured and Indigent Patients in Sacramento County • One of the most Culturally and Ethnically Diverse Communities in the USA • Training Site for both Psychiatry and Internal Medicine Residents • High Co-morbidity of Mental Illness, Substance Abuse and Chronic Medical Illness • High Rate of Hepatitis C

  6. Preliminary Outcomes • 7/2005-9/2006 • 1255 Patients Have been evaluated and Treated by the Faculty and Residents of the Program • 550 Psychiatric Consults both formal and informal have been made

  7. Sources of Patients • Primary Care Consultation From RST’s • Crisis Unit-SCMHTC • In-Patient Units of the SCMHTC • Sacramento County Alcohol and Drug • Sacramento Primary Care Clinics • Walk patients to the PCC • APSS-patients (who have lost Medi-Cal)

  8. Outpatient “Medicine-psychiatry” General Internal Medicine Rotation for Psychiatry Residents • Innovative Program to provide Psychiatry Residents training in the primary care of complex patients at the SCPCC • Supervision by combined internal medicine and family medicine faculty • Bimonthly Medicine-Psychiatry Conference by UCDMC faculty and residents

  9. Outpatient “Medicine-Psychiatry” General Internal Medicine Rotation for Psychiatry Residents • Weekly Didactics cover core topics in primary care over 2 months • “Curbside” consultation and supervision for clinicians at SCPCC • Exposure to evaluation and treatment of Hepatitis C in the Primary Care Setting

  10. “Med-Psych” Fourth Year Medical Student Rotation • Parallels General Medicine Rotation with emphasis on exposure to integrated care • Includes both inpatient and outpatient settings in addition to the PCC • 3 tracks with focus on integration of psychiatry with either internal medicine or family medicine

Accredited Training Programs Well developed training programs with experience in combined training Enthusiastic support from both departments Designated Areas for integrated training in both inpatient and outpatient 3 full time combined internal medicine and psychiatry attendings Advisory Group with faculty from all departments oversee training and education Combined Internal Medicine and Psychiatry Residency Program
