이온이동 경로 1) Pore pathway (세공 경로) 모낭과 땀샘은 피부에 수직으로 분포하며 표피 각질층에 세공 (pore)형성 세공은 각질층의 장벽과 달리 저항이 낮아 이온이나 분자가 쉽게 이동 모낭과 땀샘이 이온 및 분자의 주요 이동 Evidence : laser scanning confocal microscopy vibrating probe electrode technique (Guy, 1998). 2) Intercellular pathway (세포간 경로) • 피부에 전류가 흐르면 전기장이 이온 및 용매의 흐름과 상호작용을 하여 10㎲-10㎳동안 0.5-1 V의 막전위가 형성되면서 세포막의 재배열이 일어나 일시적으로 수용성 통로가 형성된다. • 전기전도도 및 투과성이 좋은 새로 형성된 수용성 통로는 이온이나 분자 전체의 주요 이동 통로이다 • 표피 각질세포 사이 지질 층판 세포막 극성부(polar portion)의 구조변화, 지질 층판의 폴리펩타이드 나선이 flip-flop운동으로 전압의존성 세공 형성 • b. 피부 수화 (hydration)증가 • c. 피부 저항 감소 • Evidence : 전자 및 형광분광법 (Higuchi 등, 1999) ETRLab
3) Transcellular pathway (횡세포 경로) 전류에 의해 세포막의 동요 (perturbation)에 의해 이온 및 분자가 표피 각화세포의 세포막을 가로질러 이동한다. Evidence : 전리되지 않는 콜로이드 분자를 음극쪽으로 이동시키는 피부 에 전기장이 형성되면 각질층의 수화가 증가되고 지질의 극성 머리부분이 전류에 영향을 받아 재배열되며 전류밀도가 증가할수록 지질 이중층에 물이 많아져 지질층판의 동요가 잘 유발 ETRLab
3 Mechanisms of iontophoresis Ionic effect (or Nernst-Planck effect) 전기와 이온의 상호작용으로 약물 이온에 추진력이 부가되어 수동적으로 표피 각질층의 세포막 투과를 촉진시킨다 이를 이온효과 또는 네른스트-플랭크효과라 한다. 농도차에 의해 확산하는 약물 이온에 전류를 가하면 전기반발 (electrorepulsion)에 의해 이온의 수동적 이동 유발 * 크기가 작은 이온 : 더 효과적으로 이동 전하를 띠지 않은 약물 분자 : 약물전달 강화 어렵다 * Ionization : 물질이 해리되어 전하를 띤 입자(이온)로 나누어지는 현상 electrolyte : 이온화가 일어나는 물질 * 극성에 따른 이온이동 (ion migration & polarity) 전해질용액에 직류전류를 통전시키면 이온의 이동 (ionic migration)이 일어난다. 같은 극끼리는 반발는 전기의 성질 이용 이온화된 약물을 조직안으로 이동 Evidence : a. 3 Mechanisms of iontophoresis 2) Electro-osmosis (전기삼투) 피부가 전기장의 영향을 받으면 전기삼투가 증가하여 분자의 이동이 원활해진다. 전기삼투는 하전된 세포막을 가로질러 전위차가 발생했을 때 액체가 일괄적으로 흐르는 현상 피부에 전류가 통전되면 전류의 방향을 따라 용매자체가 용적이동 (volume flow)을 한다. 이때 용매에 용해되어 있던 이온과 비이온 분자가 농도차와 관계없이 용매를 따라 이동된다. 특히 크기가 큰 음이온이나 음으로 하전된 분자량이 큰 단백의 이동에 중요하게 기여한다. 전기삼투는 전기반발과 함께 이온도입과 전기침투에 중요한 영향을 미친다. * 전기삼투는 피부의 pH에 따라 양이온과 음이온의 이동을 촉진시킨다 a. Above pH 4 사람의 피부는 pH 4이상에서는 음전하를 띠고 있다 전기삼투의 흐름은 항상 반대이온 (counterion)의 흐름과 같은 방향으로 형성 따라서 전기삼투의 흐름도 양극에서 음극으로 흐른다. b. Less pH 4 전기삼투의 흐름이 감소 전기삼투 방향이 음극에서 양극으로 반전 ETRLab
3 Mechanisms of iontophoresis 3) Increased permeability (투과성 증가) 피부에 전류를 통전 시키면 시간 경과에 따라 피부저항이 현저히 감소하고 투과성이 증가 * 투과성 증가 요인 a. 일차적 요인- 약물이온에 의한 저항감소 피부저항의 감소는 피부표면에 도달하는 약물이온과 관련이 있다. 피부표면을 점유하는 약물이온이 증가할수록 피부저항이 더욱 감소된다. 100 ㏀/㎠의 저항을 가진 피부에 전류가 흐르면서 몇 분내에 1-10 ㏀/㎠정도로 감소된다 b. 이차적 요인- 피부의 구조변화 피부에 전류가 통전 되면 이차적으로 피부 각질층의 구조변화가 일어난다. 구조변화는 부분적 비가역적으로 일어나기 때문에 이를 손상효과 (damage effect)라 하며 전류밀도가 높아지면 손상효과가 증가한다 * 투과성 증가는 비이온 분자의 이동을 강화한다. 5. 이온도입 약물의 조건 1) Ionization (이온화) 이온도입에 사용하는 화합물은 물에 해리되어 이온화가 일어나야 한다. 약물이 피부를 통과하기 위해서는 선택적 투과성을 가진 각질층의 투과장벽을 통과해야 한다. 약물이 세포막을 통과하기 위해서는 물과 지질에 용해되는 이상성(biphasic)특성이 있어야 한다. 경피전달을 위한 이상적인 화합물 지질-물 분배계수가 높고 수산기와 메틸기를 적게 가진 극성-비극성이 균형을 이루어야 한다 극성 친수성인자 : 이온화 용액을 만드는데 필요 비극성 지용성인자: 세포막을 투과에 필요 ETRLab
5. 이온도입 약물의 조건 2) Conductivity (약물의 전도도) - 이온의 전기적 특성 이온도입에는 전도도 가 좋은 약물을 선택하는 것이 바람직하다. 약물이온의 전도도는 약물의 종류와 농도에 따라 다르다. * 이차증류수에 용해시킨 0.5 mM의 NaCl을 기준 국소마취제 (lidocaine HCl, procaine HCl, cocaine HCl, mepivacaine HCL, prilocaine HCl) : 높다 국소마취제 (bupivicaine HCl) : 낮다. 혈관수축제 (epinephrine bitartrate, levarterenol bitartrate) : 높다 혈관수축제 (phenylephrine HCl) : 약간 낮다. 부신피질호르몬제 (hydrocortisone phosphate) : 높다 부신피질호르몬제 (methylprednosilone sodium succinate, hydrocortisone sodium succinate) : 약간 낮다. 항암제 (Methotrexate, cyclophosphamide) : 높다. 5. 이온도입 약물의 조건 3) 약물의 pH pH 5.0-6.0 : 이온화 최대, 전도도 가장 우수 higher pH : decreased ionized molecules, decreased conductivity 이온도입에 사용하는 약물제제의 pH 조절이 중요하다. 국소마취제의 경우 pKa가 9.0이며 용액의 pH가 9.0일 때 양이온과 유리염기가 50%씩 존재하고, pKa 7.9에서 95%가 이온화되며 이때의 pH는 약 6정도이다. 3. 생리학적 특성 (Physiological property) 이온도입에 따른 효과는 이온이동 수 (number of transferred ions), 침투깊이, 이온이 피부에서 다른 물질 또는 침전물과 화학적으로 결합하는지 여부, 이온이 모세혈관의 혈류를 타고 이동하는지 여부에 따라 다르다. 1. 이온의 침투 (ion penetration) 이상성 (biphasic) - 지용성과 친수성- 성질이 함께 있어야 한다. 친수성 또는 극성 인자 : 이온화 지용성 또는 비극성 인자 : 이온의 조직 침투 2. 전극의 전류밀도 (current density) 음극–자극성 (irritation)이 높아 피부자극 및 화상 가능성 많다 부식성 수산화 나트륨 생성, 수소이온농도 높다, 반응속도 빠르다 * 이온도입치료시 음극전극에서 전류밀도가 낮게 형성되도록 음극전극을 양극전극보다2배이상 크게 만들어 사용해야 한다. * 이온도입치료시 적절한 활성전극의 전류밀도 : 0.1-0.5 mA 또는 0.5-1 mA/ cm2 3. 이온도입의 양 전류의 강도 및 밀도, 통전시간, 이온의 농도에 따라 결정. basic formula I x T x ECE= grams of substance introduced ETRLab
2. 전극의 전류밀도 (current density) The geometric design of the electrode is an important factor for achieving an optimal iontophoretic flux without causing harms or even burns to the skin. Accurate measurements of current densities in the various skin layers and in the subcutaneous tissues are very difficult to perform in vivo and can only provide local information. Therefore computer simulations are used to model the distribution of the electric current density and to investigate electric field effects for some special electrode designs. ETRLab

Dose I x T x ECE = grams of substance introduced An iontophoresis dose is expressed in milliampere-minutes (mA-min). A mA-min is a function of current and time. Total drug dose delivered (mA-min) = current x treatment time. 40mA-min dose = 4.0mA current x 10 minutes treatment time OR 40mA-min dose = 2.0mA current x 20 minutes treatment time. A typical iontophoretic drug delivery dose is 40mA-min, but can vary from 0 to 80 mAmin.
