  • 13 Dec 2012: (Deployed) MQ-8B Fire Scout crashed during recovery to ship. (UAS)

  • 12 Dec 2012: (NAS North Island) MH-60R sustained hard landing. No fatalities.

  • 06 Nov 2012: (MCAS Miramar, CA) F-18E in-flight Physiological episode resulted in permanent total disability. (FRM)

  • 06 Jul 2013: (Norfolk, VA) E-3 standing watch topside fell overboard and failed to surface. Body recovered approximately 3 hours later.

  • 19 Jun 2013: (Guam) E-4 died while conducting work-related diving operations.

  • 07 Jun 2013: (Indian Ocean) Class C fire in #2 SSTG.

  • 26 Feb 2013: (Aberdeen, MD) During unit-level training final evaluation problem dive, two E-5's were recovered unconscious and unresponsive. One died on scene and the other diver was pronounced dead at the hospital.

  • 19 Feb 2013: (Oahu, HI) E-6 died during a planned open ocean swim evolution.

  • 14 Feb 2013: (near Catalina Island, CA) During submarine rescue system training, the Pressurized Rescue Module (PRM) fell damaging the PRM, transfer skirt, deck cradle, and umbilical.

  • 17 Jan 2013: (Philippines) MCM ran aground operating in the Sulu Sea near Tubbataha Reef.

  • 09 Jan 2013: (Twentynine Palms, CA) CH-46E sustained damage during confined area landing. No fatalities.

  • 11 Oct 2012: (Luzon, PI) CH-46E destroyed in hard landing, roll-over and fire. No serious injuries.

  • 16 Sep 2013: (Twentynine Palms, CA) E-4 died when ordnance ignited AAV during a training exercise.

  • 13 Jul 2013: (Twentynine Palms, CA) E-2 suffered a permanent and total disability while conducting artillery live fire training.

  • 18 Mar 2013: (Hawthorne, NV) Seven Marines were killed and seven Marines and one Sailor were injured in a night mortar field exercise.

  • 12 Mar 2013: (Twentynine Palms, CA) E-2 died in a bulldozer mishap while participating in an Integrated Training Exercise.

  • 26 Feb 2013: (Perris, CA) E-5 died during parachute operations training.

  • Rate values above the UCI or below the LCI infer a statistically significant change is probable. This is only an indicator. Significance cannot be determined until end-of-year.

  • Values between the UCI and LCI infer that nothing significant has occurred to increase or decrease mishap rate.

  • (Confidence Interval is based on: Poisson Rate related to a Chi Squared Distribution and calculated using FY09-13 data except for the Lost Work Days and Lost Time Case slides which were based on a normal distribution).
