Dependent Verification Deadline • November 26, 2008 was the deadline for OGB to receive all dependent verification documents • On December 31, 2008 OGB health and life coverage ends for all unverified dependents • Plan members MUST repay claim payments (OGB going to plan member to recoup and is taking care of employee premium difference) and STATE share of premiums paid for all ineligible dependents back to July 1, 2002 or date of coverage if later (OGB states they are not responsible for collecting overpayments of employer contributions). 12/11/2008

OGB Notice December 8,2008 • Each agency received a list of employees who have not provided the required documents. • Effective December 8, 2008, OGB will no longer pay health, prescription or life insurance claims for all unverified dependents. • Coverage for all unverified dependents will be terminated effective December 31, 2008. 12/11/2008

  4. One Time Submission • Coverage terminated effective December 31, 2008 • Is one of Your current active employee listed on this report from OGB? Update the record before the close of calendar year 2008, ppe 12.14.08. • Process a one time refund for deduction processed ppe 11.30.08 to avoid W-2C • Provide your OGB listing as documentation 12/11/2008

  6. Plan Member • Maybe an Active employee • Maybe a Retired employee • Maybe a Terminated employee who is in state service elsewhere • Maybe a Transferred employee who is in state service elsewhere 12/11/2008

  7. Understanding Column Headings • The cost for the coverage for the identified unverified dependent premiums. • Name of unverified dependent. • Date of Birth of the unverified dependent. • Coverage Dates – Begin and End dates of coverage of the unverified dependent. 12/11/2008

  8. OGB • Using the Coverage dates provided on the OGB Dependent Unverified and Terminated Form will be the dates used to update the employees records in PeopleSoft. • Member: Doe,John • Dependent: Doe,Child • Birth Date: 03/15/1989 • Coverage Dates: 09/01/2003 – 12/31/2008 12/11/2008

  9. Example Member Name: Doe,John • Hired 8/01/2003 • Health Coverage @ Hire Family Coverage • Doe,Child – Dependent unverified 12/11/2008

Review history rows • Determine how many history records require updating. • Place cursor on Coverage Begin Date identified Coverage Date in question. • Insert a row • Coverage Begin Date will be the same date on the history record. • Deduction Begin/End Date enter the last pay period date to stop the deduction. • Election Date: Date of OGB form or the date you entered the updated action. • Remove dependent from Dependent Tab. 12/11/2008

Update ALL Affected History Records • Fiscal Year Annual Enrollment Changes with changes in “Coverage and/or Plans” is an example of history records will to be updated. • Continue to scroll and repeat process by updating all history records for health and/or life records. • Coverage Begin Date enter 09/01/2003 • Deduction Begin/End Date 12/01/08, 1st day of the current pay period • Election Date • Remove child from “Dependent tab” 12/11/2008

  12. Final Effective Record • Last effective dated row should reflect accurate coverage per OGB. • Coverage Begin date 07/01/2008 • Deduction Begin date 12/02/08 12/11/2008
