  • For one-on-one coaching, we proactively identify members using the following eligibility criteria:

    • Health assessment reveals at least one behavioral trigger:

      • Weight

      • Tobacco

      • Stress

      • Diet

      • Exercise

    • Intention to change behavior within six months

    • Willingness to receive telephone contact

    • At least 18 years old


Healthy Lifestyles: Member Outreach

For one-on-one coaching, the health coach initiates outreach to identified members via an outbound telephone call to:

  • Follow up on the member’s HRA results

  • Establish rapport

  • Capitalize on the member’s “What’s In It For Me”

  • Confirm behavior change readiness

  • Conduct an in-depth behavior change assessment to determine participant knowledge, attitude and behaviors
