  28. Provider Mgt. – Provider Change Report • Description • A web-based report that allows the user to get a list of all the Providers whose names, license numbers, or addresses have changed. Child Mgt. – Duplicate Children Claimed Report • Description • A web-based report that lists all children who may be claimed by different Providers (for all Sponsors or within a Sponsor), based on certain matching characteristics. • Filtering Options • Sponsor • Claim Month • Sorting Options • NONE • Special Notes • The system flags a child as a duplicate if the following match: • First Initial of First Name, Last Name, & Birth Date • This report only displays children when they are claimed by two different Providers in the same month. So the report first identifies the children as duplicate between two distinct Providers, then determines any same day overlap. • Sample

  45. Meal/Attend. Mgt. –Meal Totals Report • Meal Totals Report • A report with the specific days claimed by that Provider in the month, with meal counts broken down by Tier noting what the Provider was actually paid.

  46. Meal/Attend. Mgt. – Claimed Attendance Report • Claimed Attendance Report • A report with the specific days attempted to be claimed by that Provider, noting the number of children recorded in attendance at each meal in that day.

  47. Meal/Attend. Mgt. –Claimed Food and Attendance Report • Claimed Food and Attendance Report • A report showing what was claimed by the Provider for the entire month. This includes both food and attendance, when specific food information is available. Also includes specific child names.

  50. Claim Mgt. – Provider Claim Totals Report • Description • This is a web-based report that lists all Providers claiming in a particular month. It can be filtered to examine a particular claim submission (e.g., the original claim, or a subsequent revision).
