Health Technician • Rapid City IHS Diabetes Program HealthTechnician GS-0640-07 • Introduction • This position is locatedin the SpecialDiabetes ProgramforIndians(SDPI) – Diabetes Prevention Initiative (DPI) on thecampus ofthe RapidCityService Unit(Sioux San Hospital).The purpose of the positionisto serve asa Health Technician andfunctionas amemberofthe SDPI– DPIstaffwithprimary functions beingto assiststaff withthe preparation, implementation,and documentation ofdiabetes prevention activities in the community. • Major Duties: • The Health Technician willbe responsibleforRecruitmentofparticipants intothe program. The program goal isto recruit48 participantsannuallyintotheprogram. Recruitmentactivities willtake placeinthe community, clinic, orona walk-in basis. • The Health Technician willbe responsibleforRetention ofparticipantsinthe program. GrantsManagement • and the CoordinatingCenter in DenverColoradoputaprogramgoalof100%retention. The Health Technician willdevelopaRetention Plan thatincludes activitiesthatpromote continued participation and identificationandremovalofbarriers to participation. Documentation ofattendance ofparticipants in programactivities is essential. Recoveryefforts willbemade forparticipantsthatmiss classesor appointments. • The Health Technician willeducate participants onthestandardized programcurriculumto meet the program goals of7%bodymass reduction and150 minutes ofexerciseperweek. Itwillbe the Health Technician’s responsibilityto schedule classes withtheparticipant and also contactthe Dietician toteach session 2, 3, 4, and 10. The responsibilityto teachsessions 2,3, 4, and 10 mayoccasionallybe performed bythe Health Technician to keep participantscurrentwiththe curriculumsessions. Once sessions are completed asign in sheetwith name and session completedwillbereturned tothe Data Collections Manager. • The Health Technician willorderallprogramlabs accuratelyforparticipantsin the program. • Case management is an essentialdutyfor the Health Technician. You willberesponsibleto meetweekly with participantsinthe Intensive portion ofthe program. Once the participanthas completed the 16 week • curriculumyou willschedule allannualand midyear labs, and meetwiththeparticipantmonthlyorquarterly thereafter. Works closelywith participantsandfamily members toidentifyways to increase exerciseandto develop exercise strategiesto meetexercisegoals. Encourages and motivatesparticipantsto achieve specific weight loss behaviorchange goals as outlinedinthestandardized curriculum. These meetings willbe documented and the documentation willbe given tothe Data Collections Managerforrecording. • Data willbe collectedforallactivities the HealthTechnicianinitiates. Once collecteditisthe Health Techniciansresponsibilityto transferdata toDataCollectionsManagerforrecording. • The Health Technician willassist with the identification and managementof thebehavioralreinforcementand incentive componentsforparticipants. • Participatein localteammeetings.

Evaluateand advise ontheorganization, methods,andprocedures foradministrative supportsystems suchas records,communications, directives, forms, files and documentation. • The Health Technician willassistintheplanningandimplementation ofcommunityactivities such as recruitment, nutrition classes, exercise activities, and othergrantrelated activities. • The Health Technician willreportviaelectronic mailmonthlyto the Health SystemSpecialistutilizingthe Case ManagerExcelSpreadsheetto reportallprogramactivitiesforreportingpurposes. • With the ProjectCoordinator, setupproceduresfor trackingand ensuringtimelyfollowup forparticipant activities,and otherprojectprocesses asneeded. • Assistin preparingprogress and datareports;assistsinensuringtheiraccuracyand timelinessinsubmissions forIndian Health Service. • Monitordata collectedandensure thatallforms arefilled outcompletely, accurately, and consistently. • Ensurepatientconfidentialityand HIPAA. • Participate activelyin allnational,regional,and localGrantee projectdevelopmentand trainingmeetings. • Maintain collaborative relationships with grantee team members, technicalassistance expertconsultants, tribalandcommunitypartners,and otherprojectstakeholdersto fostersuccessfulprojectdevelopmentand implementation. • The Health Technician willdevelop and coordinate CommunityBased Activities. Theseactivitiesare for diabetes prevention awarenessin ourcommunityand can include programparticipantsand non program participants. • Develop andcoordinateprogramadvertisementactivities. • Assistin the developmentand dissemination ofprograminformation. • The Health Technicianisresponsible forcleaningthe Diabetes Prevention Initiative building. • Assistin developingand adheringto written programpolicies andprocedures. • Performs otherpositionrelateddutiesasassigned. • Knowledge Requiredby the Position: • Knowledge relatedto therole thatphysicalactivityandnutritionplayinthepreventionofdiabetes. • Knowledge ofMotivation Interviewing, Stages ofChange, and othereducationalconceptsto promote behaviorchange. • Knowledge ofbasic medical terminologyto recordandretrieve medicalinformation. • Knowledge ofpropergrammar, spellingand punctuation forpurposes ofdocumentingobservationsand recordingused in the promotion ofservices. • Knowledge oftheregulations on confidentialityand patienteligibility. • Familiarityofcommunityresources. • Abilityto plan,coordinate,scheduleand evaluate properactivities within the community. • Abilityto communicatecourteouslyand effectivelywith patients,theirfamilies,medicaland administrative staff;health care professionalsand the generalpublic. • Abilityto communicateboth verballyand in writing. • Abilityto use a varietyofaudio-visualequipment. • Supervisory Controls: • The incumbentis underthedirectsupervision of the DiabetesPrevention Initiative, Health SystemSpecialist, who providesspecific oraland/orwritteninstructions.The supervisordefinestheobjectives, priorities, and deadlines forprojects orassignments and assiststheemployee with unusualsituations orproblems thatdonot

  3. have clearprecedents. Thesupervisorsestablishes workschedules forthe program, providesguidance, direction and instruction inperformingcomplex duties. Otherduties as assigned,to meetthe programand patientneeds. Workwillbereviewedbysupervisorforoverallprogrameffectivenessin meeting grantobjectives. Guidelines: Guidelines include IndianHealth Service policies andregulations;JCAHOstandards, Tribal, Stateand Federal laws and policies;RPMS/PCCmanuals;PrivacyAct Guidelines HIPAAregulations, Service Unitdirectivesand precedents;oraldiscussionwiththe Director/Coordinatorofthegrant. Otherguidelines include StandardofCare forDiabetics and others asdirected byGrants Management. Guidelines are applicablein most, butnotevery situation. The incumbentusesjudgmentto selectthe guideline whichis appropriate forthespecific situation.Problems areto be referred tothesupervisororotherdelegated authorityin charge whenexistingguidelines cannotbeapplied. Complexity: The workconsists ofprovidingservices to diabetic patients in the area ofexerciseand properbehaviorregarding good health practices.Workwill involve a varietyofprocesses and methods. Decisions are based upon the employee's knowledge ofduties, priorities, policies,and programgoals ofthe specific areaofperformance. Scope andEffect: The workconsists ofprovidingdirectionand assistance to participants inpromotingphysicalactivityand nutrition. The workhas adirectand criticalimpactontheprovision ofqualityservicestothepatientpopulation served. Competencyand accuracyaffects patientand programoutcomes. The goalis theenhancementand promotion of the health and wellbeingofpatients. PhysicalDemands: Workrequires physical fitness workwith some physicalexertion such asprolonged periods ofwalking, standing, bending, reaching, crouching, stooping, pushing, pulling, stretchingand liftingof trainingmaterials/equipment/ supplies. Attimes the workloadcan become veryhectic and stressful. Incumbentsubjecttocoverrotationalshifts (days/evenings/nights), holidays and weekends. This position requiresoperation ofa motorvehiclefortravelto variouscommunity meetingplaces. Avalid driver's licenseisrequired. Work Environment: Workis performed inthe Prevention Buildingon theRCSU Campus, in communitybuildings/communitysettings, outdoorsettings, and inclinics. Potentialhealth hazardsencountered duringthe provisions ofservices include exposureto infectiousdisease agents and while usingcleaningchemicals.
